Carolina Administrative Code (15A NCAC 2H.0105 (e)) requires that an application for

renewal be filed at least 180 days prior to expiration of the current permit. To
satisfy this
requirement, your renewal package should have been sent to the Division postmarked
no later than
January 1, 2011.
The Division has not received a renewal request for the subject permit. This is a
violation of your
permit at Part II. B. 10., which states "Any permittee that has not requested
renewal at least 180
days prior to expiration... will subject the permittee to enforcement procedures as
provided in NCGS
143-215.6 and 33 USC 1251 et. seq.".
In order to prevent an assessment of civil penalties you must submit a completed
permit application
(see enclosed forms) no later than March 4, 2011. use the enclosed checklist to
complete your
renewal package. The checklist identifies the items you must submit with the permit
renewal
application.

The Division has reviewed your letter of April 22, 2010, which states the subject facility is not producing
any wastewater discharge; the treatment system has been inactive since October 2009.
After considering the recommendations of the staff in the Winston-Salem Regional Office, I have agreed to
waive the requirement to submit monthly DMRs for the subject facility, effective May 1, 2010 The DMRs
up through (and including) April 2010 should be submitted as usual.
Be advised that if and when any wastewater discharge resumes, the discharge must be monitored as per
the terms of the NPDES permit and other applicable rules or statutes. Please keep the Winston-Salem
Regional Office apprised of any pertinent developments that arise regarding this facility, especially if the
discharge is re -activated.
While requirements for operator visitation, monitoring and reporting are being waived, your company is
still required to pay the Annual Administering and Compliance Monitoring Fee that is associated with this
and all NPDES permits.

Governor Bev Perdue recently signed legislation to complete passage of rules for the Jordan Lake
Nutrient Management Strategy. The Strategy includes a Wastewater Discharge rule (T15A NCAC 02B
.0270) that applies to your wastewater treatment facility. Pursuant to Sub -Item (6)(d) of the rule, I am
writing to notify you of the nutrient allocations assigned to your facility.
This letter addresses only certain provisions of the Jordan Lake Wastewater Discharge rule. I encourage
you to familiarize yourself with the remainder of this rule.
Nutrient Allocations. The Wastewater Discharge rule establishes the maximum loads of nitrogen and
phosphorus that can be released from wastewater treatment facilities into the Jordan Lake watershed.
The rule provides that these wasteload allocations are to be divided among the existing facilities in
proportion to (with specific exceptions) their 2001 permitted flows. It also provides that facilities
permitted to discharge 100,000 gpd or more are to receive nitrogen and phosphorus limits equivalent to
their individual allocations.
Your facility has been assigned nitrogen and phosphorus allocations. The allocations are expressed as
annual mass loadings delivered to the lake. Due to foreseeable losses between the discharge point and
the lake, they are also expressed as the equivalent discharge loads at the point of discharge. The
delivered and the corresponding discharge allocations assigned to your facility are as follows:

Notification of Jordan Lake Nutrient Requirements

Nutrient Limits. Your facility will not receive new nutrient limits at this time. However, any nutrient
limits already in your NPDES permit will remain in effect.
If the facilities permitted for less than 100,000 gpd, as a group, approach their allocations for nitrogen
or phosphorus, the Division will consider whether nutrient limits are necessary to protect water quality
at that time. In the event that nutrient limits become necessary, the limits would equal your facility's
discharge allocations.
Nutrient TradinZ. The rule provides that, when one utility connects to another, the allocations held by
the closing facility are transferred to the remaining facility. The rule also provides for the trading of
allocations among dischargers within each of the major Jordan subwatersheds. Be sure to consult with
the Division's NPDES staff before you consider selling allocation: transactions can have profound
implications for your facility and its permit.

A fast track application for Authorization to Construct dechlorination facilities was
received on December 13, 2007, by the Division. Authorization is hereby granted for the
construction of modifications to the existing Guildford Correctional Center WWTP, with
discharge of treated wastewater effluent into North Buffalo Creek in the Cape Fear River Basin.
This authorization results in no increase in design or permitted capacity and is awarded
for the construction of the following specific modifications:
Installation of a dechlorination system utilizing sodium sulfite pursuant to the fast
track application received on December 13, 2007, and in conformity with the
Minimum Design Criteria for Dechlorination Facilities.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of
NPDES Permit No. NCO029726 issued May 5, 2006, and shall be subject to revocation unless
the wastewater treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0029726.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.

Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This final permit includes changes from the draft permit sent to you on April 1, 2006. The NH3
limits in the drat were incorrect as a result of calculation errors. These have been corrected in the final
permit.
This permit includes a TRC limit that will take effect on ,lanuary 1, 2008. If you wish to install
dechlorination equipment, the Division has promulgated a simplified approval process for such projects. Guidance
for approval of dechlorination projects is attached.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable
to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt
of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh,
North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal requirements
to obtain other permits which may be required by the Division of Water Quality or permits required by the
Division of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit

Division personnel have reviewed and approved your application for renewal of the subject permit. Accordingly, we are
forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency
dated May 9,1994 (or as subsequently amended). The limits and requirements in this permit will take effect on July 1, 2001.
Please be aware that North Buffalo Creek is listed as an impaired stream on North Carolina's 303(d) list. This means that
the stream does not meet all water quality standards. The Division is working toward the improvement of water quality in North
Buffalo Creek. If the Division's current initiatives do not result in improved water quality, removal of discharges.to North
Buffalo Creek may be necessary for the health of the stream. Although the Division believes larger discharges and urban runoff
are the primary causes of degradation to North Buffalo Creek, we will continue to monitor compliance at your facility. If
noncompliance with your permit's effluent limits is shown to be a direct cause of stream degradation, the Division will
recommend removal of your facility's discharge.
Your facility discharges to a nutrient -sensitive water (NSW). In 1997 the Clean Water Responsibility Act (House Bill 515)
was enacted, with the intent to strengthen the control of nutrients from all discharges to NSW waters. Facilities permitted to
discharge greater than 500,000 gallons per day (and any new or expanding facilities) will be subject to nutrient limitations for total
nitrogen and total phosphorus according to House Bill 515 requirements. Since modifications to treatment systems are expensive
and time consuming, it is highly recommended that you begin planning for this possibility now if a future expansion is anticipated
at your facility.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have
the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must
be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is
made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require modification or
revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area Management
Act or any other Federal or Local governmental permit that may be required. If you have any questions concerning this permit,

Dear Mr. Stovall:
The Division is beginning the cycle of permit renewals for the Cape Fear river basin. An examination of the Cape
Fear basin plan and the NPDES permit schedule has revealed that the existing expiration dates in Cape Fear NPDES
permits do not coincide with the revised permit expiration dates for the basin. This discrepancy could create unnecessary
delays in the processing of over 200 permit renewals. Accordingly, the Division is changing the permit expiration dates for
NPDES permits in the Cape Fear river basin.
This permit modification changes the expiration date of the subject permit in subbasin 30602 to June 30, 2001. Please
find enclosed the revised permit cover page. Insert the new cover page into your permit and discard the old page. All
other terms and conditions contained in the original permit remain unchanged and in full effect. This permit modification
is issued under the requirements of North Carolina General Statutes 143-215.1 and the Memorandum of Agreement
between North Carolina and the U. S. Environmental Protection Agency.
In accordance with the modification of the permit expiration date, the new due date for your renewal application is
January 1, 2001. Renewal application forms will be sent to you approximately 0-8 weeks in advance of the new due date
or you may download the forms from the NPDES web site at http://h2o.enr.state.nc.us/NPDES/documcnts.html.
You will need to download Short Form D. If you have already submitted a permit renewal form, there is no need to
submit any other forms to the Division; the application will be processed during the renewal period for your subbasin.
If any part of this permit modification are unacceptable to you, you have the right to an adjudicatory hearing upon
written request within thirty (30) days following receipt of this letter. This request must be a written petition conforming

and found to be satisfactory. Authorisation is hereby granted for the construction
of modifications to the existing 14,500 CPD septic tank - surface sand filter treat-
ment facility consisting of a 14,500 CPD intended aeration package plant with a
15,000 gallon aeration basin, final clarification, and sludge facilities, post
chlorination facilities, post saration facilities, rehabilitation of the existing
surface sand filters and abandoning the existing 12,000 sallow septic tank to serve
the 1. C. Department of Corrections Guilford II Subsidiary.
This is a Class 11 Wastewater Treatment Facility and the person in responsible

Division staff has confirmed that, the subject Certificate of Coverage is no longer required. Therefore, in
accordance with your request, NPDES Permit NCG510210 is rescinded, effective immediately.
If in the future your company wishes to discharge wastewater to the State's surface waters, it must first
apply for and receive a new NPDES permit. Discharge of wastewater without a valid NPDES permit will
subject the responsible party to a civil penalty of up to $25,000 per day.

Description of Treatment Process, Collection System, and Facilities Served:
Treatment Process — The treatment facility is a United Systems Biological Contractor Unit.
The unit is basically a multi -stage complete mix biological treatment unit, which utilizes
considerable recirculation and diffused air. The unit consists of four (4) corner compartments,
and inner and outer ring, and an effluent well. The unit also contains UV disinfection and
continuous flow measurement. A cloth filter media vertically dissects the outer ring. The
unit is used in conjunction with septic, tanks that are present on the five homes this unit
serves. The treated effluent is discharged to the Horsepasture River in the Savannah River
Basin.
Collection System — The collection system is a standard design flowing from the individual
septic tanks on the system. The system was designed to meet all state standards for wastewater
collection systems.
Facilities Served — This system serves five (5) homes in the Cedar Creek Woods Development.
II. WWTP Performance
All operating parameters were in compliance with NPDES permit limits for 2008, except for the
following occurrences. In July 2008, there was a fecal coliform violation.
We take pride in our efforts to protect the local environment and the quality of waters in
our streams.
III. Collection System Performance
The collection system experienced no significant problems for the reporting period.

Permit was signed on September 20th, 2011 and took effect October 1, 2011. This renewed CoC is issued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina -and the US Environmental Protection agency dated October 15, 2007 [or
as subsequently amended].
A copy of the NCG510000 is enclosed, as well as an updated Technical Bulletin. The Technical
Bulletin summarizes the changes from the previous version of NCG510000. If any parts, measurement
frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to
request an individual permit by submitting an individual permit application. Unless such demand is made,
this CoC shall be final and binding.
The CoC is not transferable except after notice to the Division. You must notify the Division if you sell
or otherwise transfer ownership of the permitted facility. The Division may require modification or reissuance
of the CoC.
If your facility ceases discharge of wastewater before the expiration date of this permit, contact the
Regional Office listed below: Once discharge from your facility has ceased, this CoC may be rescinded. This
permit does not affect the legal requirements to obtain other permits which may be required by the Division of

Your facility is currently covered for wastewater discharge under General Permit NCG510000. This permit expires
on September 30, 2001. We are currently in the process of rewriting this permit and are scheduled to have the
permit reissued by the summer of 2001. Once the permit is reissued your facility would be eligible for continued
coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will be expiring. Enclosed you will find a general permit coverage renewal application
form. This will serve as your application for renewal of your permit coverage. The application must be
completed and returned with the required information by February 28, 2001 in order to assure continued coverage
under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility
without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10,000 per day.
If the subject groundwater remediation project has been terminated, please complete the enclosed rescission
request form. Mailing instructions are listed on the bottom of the form. You will be notified when the rescission
process has been completed.

diesel, fuel oil, and kerosene) as well as gasoline contamination. Please note that the permit has three effluent
limitations pages. The type of contamination at your site (gasoline only, heavy fuels only, or gasoline and heavy
fuels) will determine the effluent limitations for your discharge.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you
have the right to request an individual permit by submitting an individual permit application. Please take notice
that the previously issued Certificate of Coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the Certificate of
Coverage.

followed in case of change of ownership or control of this discharge.
The Authorization to Construct is issued in accordance with Part III, Paragraph 2 of NPDES
Permit No. NCG510000, and shall be subject to revocation unless the wastewater treatment facilities are
constructed in accordance with the conditions and limitations specified in Permit No. NCG510000.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required by this
Division, such as the construction of additional or replacement wastewater treatment or disposal facilities.
The Winston-Salem Regional Office, telephone number 910/896-7007, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be
made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00
a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.

Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been installed in
accordance with the NPDES Permit, the Certificate of Coverage, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box
29535, Raleigh, NC 27626-0535.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for the
life of the facility.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Environmental Management in accordance with
North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be required by the Division of Environmental
Management or permits required by the Division of Land Resources, the Coastal Area Management Act or

The groundwater treatment system at the facility does not currently have a
sludge management plan, due to the fact that `sludge' is not produced by the
system.
The only solids that could be said to be indirectly produced by the system are the
carbon canisters which are properly recycled by the subcontractor that services
the facility. This carbon is recycled via thermal processes that in effect `recharge'
the carbon for reuse. Approximately 100 pounds of used carbon is recycled per
year.

The Division is beginning the cycle of permit renewals for the Cape Fear river basin. An examination,of the Cape
Fear basin plan and the NPDES permit schedule has revealed that the existing expiration dates in Cape Fear NPDES
permits do not coincide with the revised permit expiration dates for the basin. This discrepancy could create unnecessary
delays in the processing of over 200 permit renewals. Accordingly, the Division is changing the permit expiration dates for
NPDES permits in the Cape Fear river basin.
This permit modification changes the expiration date of the subject permit in subbasin 30608 to August 31, 2001.
Please find enclosed the revised permit cover page. Insert the new cover page into your permit and discard the old page.
All other terms and conditions contained in the original permit remain unchanged and in full effect. This permit
modification is issued under the requirements of North Carolina General Statutes 143-215.1 and the Memorandum of
Agreement between North Carolina and the U. S. Environmental Protection Agency.
In accordance with the modification of the permit expiration date, the new due date for your renewal application is
March 4, 2001. Renewal application forms will be sent to you approximately 6-8 weeks in advance of the new due date or
you may download the forms from the NPDES web site at http://h2o.enr.state.nc.us/NPDES/documents.html. You
will need to download Short Form C-GW. If you have already submitted a permit renewal form, there is no need to
submit any other forms to the Division; the application will be processed during the renewal period for your subbasin.
If any part of this permit modification are unacceptable to you, you have the right to an adjudicatory hearing upon
written request within thirty (30) days following receipt of this letter. This request must be a written petition conforming
to Chapter 150B of the North Carolina General Statutes, filed with the Office of Administrative Hearings (6714 Mail
Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.

This discharge was not eligible for coverage under NCG510000 because the discharge is to
WS waters and the specified minimum treatment requirements would necessitate the
addition of an oil and water separator to the treatment system. The source of groundwater
contamination is a petroleum release from an underground storage tank formerly located
on -site. Preliminary investigations revealed that the contamination is dissolved phase
petroleum product contained on -site in a 55 ft. by 62 ft. plume. The remediation system
will consist of 5 recovery wells, a water meter, alarm system, STAT 15 low profile air
stripper, and activated carbon adsorption to treat 14,400 gpd (10 gpm). The effluent will
be piped to the on -site storm drain system for discharge.
Sampling results were provided for one monitoring well located in direct vicinity to the
former 3,000 gallon gasoline UST designated as the source of contamination. A copy of
the submitted data is attached. Reported treatment system removal efficiencies are in excess
of 99% for Benzene, Toluene, Ethylbenzene, and Xylenes. Predicted effluent
concentrations based on removal efficiency calculations (attached) show that monitoring is
required for Toluene, Benzene, Ethylbenzene, Lead, and Phenols. Proposed monitoring
requirements shall also include TSS due to reported concentrations of 2,750 mg/1 in MW-
13. MTBE monitoring is also recommended.

A letter of request for an Authorization to Construct was received on October 27, 1994 by the
Division and final plans and specifications for the subject project have been reviewed and found to be
satisfactory. Authorization is hereby granted for the, construction of a groundwater remediation system
consisting of an inlet flow regulating manifold, low profile air stripper, repressurization pump, filter, and
two granular activated carbon units in series.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES
Permit No. NCO084492 issued January 17, 1995 and shall be subject to revocation unless the wastewater
treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No.
NC0084492.
The sludge generated from these treatment facilities must be disposed of in accordance with G.S.
143-215.1 and in a manner approved by the North Carolina Division of Environmental Management.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required by this
Division, such as the construction of additional or replacement wastewater treatment or disposal facilities.
The Winston-Salem Regional Office, telephone number [phone] shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be
made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00
a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.
Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been installed in
accordance with the NPDES Permit, this Authorization to Construct and the approved plans and
specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box 29535, Raleigh, NC
27626-0535.
Upon classification of the facility by the Certification Commission, the Permittee shall employ a

facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the
classification assigned to the wastewater treatment facilities by the Certification Commission. The
Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with
the conditions of Title 15A, Chapter 8A, .0202. The ORC of the facility must visit each Class I facility at
least weekly and each Class II, III, and IV facility at least daily, excluding weekends and holidays, and
must properly manage and document daily operation and maintenance of the facility and must comply with
all other conditions of Title 15A, Chapter 8A, .0202. Once the facility is classified, the Permittee must
submit a letter to the Certification Commission which designates the operator in responsible charge within
thirty days after the wastewater treatment facilities are 50% complete.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for the
life of the facility.
The Operational Agreement between the Permittee and the Environmental Management
Commission is incorporated herein by reference and is a condition of this Permit. Noncompliance with the
terms of the Operational Agreement shall subject the Permittee to all sanctions provided by G. S. 143-
215.6 for violation of or failure to act in accordance with the terms and conditions of this Permit.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Environmental Management in accordance with
North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from complying
with any and all statutes, rules, regulations, or ordinances which may be imposed by other government
agencies (local, state, and federal) which have jurisdiction.
