Rt. on 2nd paved driveway between two houses. Residence is at end (approx. 1 /4 mile) 6. Discharge Point- Latitude: 36° 27' 00" Longitude: 79045' 12" Attach a USGS Map Extract and indicate treatment plant site and discharge point on map. USGS Quad No.: B2ONW and USGS Quad Name: SE Eden 7. Size (land available of expansion and upgrading): Area should be adequate but may require R&R for any repairs. • 8. Topography (relationship to flood plain included): Not in flood plain. 9. Location of nearest dwelling: Next-door residence. 10. Receiving stream or affected surface waters: UT Town Creek a. Describe receiving stream features and pertinent downstream uses: Creek flows through residential developments with several SFR discharges. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1 . ST, 261 sq. ft. subsurface sandfilter, chlorinator, and discharge pipe. 5. Sludge handling and disposal scheme: • Pumped and hauled as needed by a licensed septage hauler. 6. Is this facility being constructed with Construction Grants Funds (municipals only)? 2. Special monitoring requests: Chlorine usage reports? 3. Additional effluent limits requests: 4. Other: System seems to be adequate. There is a question as to whether or not chlorine disinfection is req'd by the permit. This needs to be clarified and the owner should be notified of any changes in the permit. The owner stated that he added chlorine; but, it was not required to his knowledge. PART IV - EVALUATION AND RECOMMENDATIONS WSRO recommends the permit be renewed with chlorine disinfection as a permit requirement.
