Herring) at the abovementioned address in Randolph County, North Carolina. You were present for the inspection. This type of inspection consists of two basic parts: a review of self-monitoring data, and an on-site inspection of the facility and its discharge. You hold General Permit NCG550164 to discharge Domestic Wastewaters from Single Family Residences under the National Pollutant Discharge Elimination System (NPDES). During the inspection, it was established that you have not been performing the self-monitoring requirements for this permit. According to permit text Part II Section B, "the permittee must comply with all conditions of this permit. Any permit noncompliance constitutes a violation of the Clean Water Act is grounds for enforcement action; for permit termination, revocation and reissuance, or modification; or denial of a permit upon renewal application." Under state law, a daily civil penalty of not more than twenty-five thousand dollars ($25,000) per violation may be assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of a permit. [Ref: NC General Statutes 143-215.6A] A copy of the General Permit was provided to you during the inspection. Permit text Part I Section A specifies the limits and monitoring requirements for your permit. The following parameters must be monitored on an annual basis: Flow, Biochemical Oxygen Demand (BOD), Total Suspended Residue, Fecal Coliform, and Total Residual Chlorine. A Laboratory with North Carolina Wastewater Certification must perform this monitoring. A list of certified laboratories was provided to you during the inspection. Please note that a path to the discharge pipe must be cleared to enable monitoring of the effluent. It is requested that you respond to this office within 15 business days of receipt of this Notice. Ms. Tina M. Herring was the previous owner. 'The house remained vacant for several years prior to Mr. Eberhart ownership. Mr. Eberhart has sent the appropriate paperwork to DWQ/Raleigh for a permit name change. Since Mr. Eberhart took ownership of the property, no repairs or changes have been made to the system. He has been regularly adding chlorine tablets to the chlorinator. Chlorine tablets were observed during the inspection. Permit Yes No NA NF (If the present permit expires in 6 months or less).
