Freeboard was less than 2 ft on 5/16, 5/17, 5/20, and 5/21/24 due to heavy rainfall contributing to higher WWTP flows. Additionaly, a low-lying liftstation was found to be underwater during a heavy rain event.
The owner was notified and a riser was added to the liftstation opening to mitigate infiltration through this liftstation.

The Monthly average flow exceeded the permitted flow of 24,400 gpd. The permitted application rate was not exceeded. The flow meter was calibrated on June 27, 2023 and the meter was reading significantly
low before the calibration. This was noted on the June 2023 NDMR. The plant is in the process of an upgrade to increase the capacity as well as investigating extraneous flow for 1/1 and leaking fixtures. Lab
Qualifiers, 5/7/2024, Fecal Coliform, D6 - The precision between the sample and sample duplicate exceeded laboratory control limits and U2 - Colonies are too numerous to count. Actual result may be greater
than reported. BOD, 5-Day, B3 - The dissolved oxygen depletion of the dilution water blank exceeded 0.2 mg/L and L2 - Analyte recovery in the laboratory control sample (LCS) was below QC limits. Results for
this analyte in associated
samples may be biased low.

QUALIFIERS
Project: FOREST LAKE -MONTHLY
Pace Project No.: 92729251
DEFINITIONS
DF - Dilution Factor, if reported, represents the factor applied to the reported data due to dilution of the sample aliquot.
ND - Not Detected at or above adjusted reporting limit.
TNTC - Too Numerous To Count
J - Estimated concentration above the adjusted method detection limit and below the adjusted reporting limit.
MDL -Adjusted Method Detection Limit.
PQL - Practical Quantitation Limit.
RL - Reporting Limit - The lowest concentration value that meets project requirements for quantitative data with known precision and
bias for a specific analyte in a specific matrix.
S - Surrogate
1,2-Diphenylhydrazine decomposes to and cannot be separated from Azobenzene using Method 8270. The result for each analyte is
a combined concentration.
Consistent with EPA guidelines, unrounded data are displayed and have been used to calculate % recovery and RPD values.
LCS(D) - Laboratory Control Sample (Duplicate)
MS(D) - Matrix Spike (Duplicate)
DUP - Sample Duplicate
RPD - Relative Percent Difference
NC - Not Calculable.
SG - Silica Gel - Clean -Up
U - Indicates the compound was analyzed for, but not detected.
Acid preservation may not be appropriate for 2 Chloroethylvinyl ether.
A separate vial preserved to a pH of 4-5 is recommended in SW846 Chapter 4 for the analysis of Acrolein and Acrylonitrile by EPA
Method 8260.
N-Nitrosodiphenylamine decomposes and cannot be separated from Diphenylamine using Method 8270. The result reported for
each analyte is a combined concentration.
Reported results are not rounded until the final step prior to reporting. Therefore, calculated parameters that are typically reported as
"Total" may vary slightly from the sum of the reported component parameters.
Pace Analytical is TNI accredited. Contact your Pace PM for the current list of accredited analytes.
TN -The NELAC Institute.
ANALYTE QUALIFIERS
B3 The dissolved oxygen depletion of the dilution water blank exceeded 0.2 mg/L.
D6 The precision between the sample and sample duplicate exceeded laboratory control limits.
L2 Analyte recovery in the laboratory control sample (LCS) was below QC limits. Results for this analyte in associated
samples may be biased low.
u2 Colonies are too numerous to count. Actual result may be greater than reported.
