In response to your letter of December 12, 2007, please find enclosed three (3) additional copies
of the application fora 401 Water Quality certification for the above-referenced project. Also
enclosed is a check in the amount of $570 for the application fee.
This project is located near Lewisville, NC and is anticipated to be under constnaction within the
next 5 months, The proposed stream crossings are over an intermittent stream and a perennial
stream, both as designa#ed on Vienna and Clemmons, North Carolina USGS Quadrangle sheets
and the total linear impact is expected to be 230 ft.
The US Army Corps of Engineers has already issued a NW 39 Permit. Please contact me if you
