Take Hwy 29 & 70 south out of Thomasville. Turn left onto
SR 1022 and travel east approximately 1 . 4 miles to Upper
Lake Rd ( SR 2024 ) . Turn right and travel east 1 . 5 mile to
Greentree Rd (SR 2751 ) . The Vaughn SFR is the third house
on the right past SR 2751 .

PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

existig) : The existing system consists of a septic tank,
391 ft subsurface sandfilter, chlorinator ( inoperable) ,
contact tank and discharge pipe.

Solids should be removed from the septic tank as necessary
by local septic tank haulers for disposal in the nearest
POTW.

this permit, the permittee shall repair the chlorinator
tubes and then begin using chlorine tablets to properly
disinfect the wastewater. In addition, all sink drains from
the residence must be tied into the septic tank sandfilter
system:`

PART IV - EVALUATION AND RECOMMENDATIONS
During the site evaluation the exact location of the
chlorinator could not be ascertained since Mr. Vaughn had removed
the tubes from the middle of his back yard to make mowing easier.
In addition, one of the sinks from the residence was straight
piped to the ditch. Mr. Vaughn was advised that both of these
deficiencies must be corrected.
WSRO recommends reissuance of the SFR permit provided that
the above clause is included.
