Everything looks good except for a few small issues: Please correct the following: • Please correct the following items with the certification forms (required per Part C.6. of the Transfer Application and 15A NCAC 02H .1045(2)(f) &.1042(4)(a)): o As-Built Permittee Certification: ■ Deed Restrictions/BUA Records, Items 1-3—Deed restrictions are not required for single lot commercial projects. Please change these items from "Y"to "N/A". ■ Operation and Maintenance Agreement, Item 1—The O&M Agreement does not appear to be referenced on the plat(Bk: 43, Pg: 296).The O&M Agreement is only required to be referenced on subdivision plats. o As-Built Designer's Certification for General MDCs,Treatment Requirements, Items 1 &2—An SCM is required to meet either runoff treatment or runoff volume match (they are mutually exclusive).
