However, the Division requires additional information before completing our review. Please address the items on the attached pages no later than the close of business on July 19, 2024. Please be aware that the Applicant is responsible for meeting all requirements set forth in North Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the review of the subject application package. The Division may return the application as incomplete pursuant to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in Sections A through K or the Applicant fails to provide the additional information on or before the above - requested date. Please reference the subject application number when providing the requested information. The Applicant shall sign, seal, and date (where applicable) all revised and/or additional documentation and submit an electronic response to my attention via the Non -Discharge online portal. Overall: 1. No comments. B. Application (Form: DCAR 06-16): 1. The signature authority in Item 1. 1. Mr. Joseph Ward is not listed in our system as an authorized signature authority to sign on behalf of the Permittee. In accordance with 15A NCAC 02T .0106(a), permit applications shall be signed by a principal executive officer of at least the level of vice- president or other authorized employee. To meet permit application submission requirements, one of the following actions shall be taken: a. Provide an updated application form signed by an authorized permission pursuant to 02T .0106(b). b. Mr. Ward shall be provided authority to sign permit applications in accordance with 15A NCAC 02T .0106(c). This delegation of authority shall be provided in writing to the Division by an authorized person, in this case a principal executive officer of at least the level of vice- president pursuant to 02T .0106(b). A copy of a signature authority delegation letter is provided with this letter. C. Program Information: 1. No comments. D. Operation & Maintenance Plan: 1. In accordance with Item C in the DCAR 06-16 Instructions form and 15A NCAC 02T .1110, the O&M Plan shall include spill control provisions. A spill response plan shall be submitted, which includes the following information as required by 02T .I 104(a)(4): a. Responses to upsets and bypasses, including control containment and remediation; and b. Contact information for program personnel, emergency responders, and regulatory agencies. E. Application (Form: RSC 06-16): 1. The hazardous waste determination for the TWSA Plant #1 in Item I1.3 does not include a result for methyl ethyl ketone. A result for this parameter shall be provided for review. Please note that this parameter may also be listed as 2-Butanone on a lab report. 2. The plant available nitrogen (PAN) information table in Item 1I.5.e of the RSC for the TWSA Plant #1 was not fully completed. This table shall be completed. 3. How was the PAN value of 8,840 mg/kg in Item 1I.5.e. determined? Based on the nutrient results reported in Item I1.5.b. and the mineralization rate in II.5.c., the first year PAN for a surface application should be 3,421 mg/kg. No comments. J. Laboratory Analytical Reports: 1. In accordance with Item C in the RSC 06-16 Instructions form, laboratory analytical reports and operational data used to complete the certification shall be included. The lab reports used to complete the Residuals Quality Information in Sections II.3 through II.5 shall be submitted for review. K. Pathogen Reduction (PR) & Vector Attraction Reduction (VAR) Compliance Documentation: 1. In accordance with Item C in the RSC 06-16 Instructions form, documentation that the facility complies with pathogen reduction and vector attraction reduction (VAR) requirements shall be included. Data that provides pathogen/VAR compliance shall be submitted for review.
