The permittee is ready to request an invoice fort he Master Infrastructure B phase (2) which for 0.02 acres of
wetland. Can you please update the NCDMS CMRT forms and provide one for the 0.02 acres. Mitigation through
NCDMS and Kelly is requesting that "(2) Master Infrastructure B" be added to the CMRT form.

Per your request, please find attached an updated CMRT form for DMS provided mitigation for the above noted
project. The only change to the form was to provide two separate forms, one for wetland credits and one for stream

We did request, and we briefly discussed the condition after approval (2nd attachment) and we included a table with
timelines. First attachment includes the final after the RFAI.
OK, let me see if Kelly Williams can create an invoice without this change, I know that she will ultimately need the
updated CMRT form to sign with the correct information.

Hi Debbie,
Did you specifically request phased mitigation, and I messed up and didn't include it in the 401? If the answer is yes, I'll
fix that. If the answer is you didn't request it, then you'd need to. I'd need an official cover letter and a clear indication
of which impacts will be in which phases and then I can modify the permit. Be warned that I'm totally swamped for the
next 2 weeks so it won't be a fast turnaround, I'm sorry.

The permittee is ready to pay mitigation for the first phase of this project. The Corps allowed mitigation phasing per the
construction on this project and they are ready to make payment on the Master Infrastructure A which is only stream
mitigation. They have paid the portion of stream mitigation to Wildlands, see attached signed CMRT form. However the
CMRT form for the NCDMS, also includes wetland mitigation (i.e. 0.35 acres). Is it possible to update the NCDMS CMRT
form and remove the wetland mitigation and put in on a different form?
Once the stream mitigation is paid for this phase, I believe that is it for the stream mitigation and NCDWR, however the
wetland mitigation is broken into 6 total phases (i.e. Master Infrastructure B, LEN B, and West Village A, B, C and D). I
don't know how the construction schedule is going to move forward so I am thinking for now, maybe just create that
new CMRT form for the 0.35 acres and then in the future we can deduct as needed and create new forms? Or you could
create all of the new forms now, but the NCDMS wetland mitigation is approximately % of what the USACE is going to
require (i.e. 0.73 acres) so I wouldn't know how to break it down per the construction/mitigation phases to divide into
updated CMRT forms.
Compensatory Mitigation Condition #21 within IP (page 14 if 124):
COMPENSATORY MITIGATION
1. In order to oompensate for impacts associated with this permit_ mitigation small
be provided in accordance with the provisions outlined on the most recent version of the
attached Compensatory Mitigation Responsibility Transfer Farm_ The requirements of
these tangs, including arry special conditions listed on the forms, is hereby incorporated
as special conditions of this permit.
Construction within jurisdictional areas on the property shall begin only afterthe
perrnittee has made payments per the below mitigation table to either an approved
mitigation bank or the NC D IVIS within the service area and providled a written
confirmation to the Corps that it agrees to accept responsibility fear time mitigation work
required_
The cast of credits will be the current price at the time of payrneK root the current price
at the time of permit issuance. No work on any phase of the project shall begin until the
