Kevin,
With Fayetteville PWC's submittal Friday afternoon of theirvariance request which is related to your client's
subject pump station and force main project, I have spent sometime today reviewing this old project file.
I believe the only item remaining from the original review comments is a new FTSE from Fayetteville PWC. Please
submit an updated FTSE and we will work to move this project forward also once a final decision is made on
Fayetteville PWC's variance request for the existing DAK 12-inch force main, that your project proposes to connect
to for sewer service foryour client's existing operation at the Wellman Plant site. Thanks, Dale.

Thanks Kevin. As you noted below, we still need an updated FTSE form for the sewer extension permit application and of
course, resolution of the Division's concerns regarding the existing 12-inch force main by Fayetteville PWC in order to
move this project forward.
However, the additional information you submitted in response to the Division's other comments below appears to be
an adequate response at this time. A final review will be conducted once all of the information is available.

Dale, we are waiting resolution of the 12" force main issue between PWC and the State, the letter of acceptance for the
Pump and Haul from PWC, and the FTSE for the sewer permit from PWC. Attached are your comments and responses
along with supporting documents and revised plans. We'd appreciate your review of these items to see if any issues
remain outstanding beside the three listed above. Please call me on my cell if you have any questions.

See the information I have attached to this e-mail.
The original pump'n haul permit issued by the NCDEQ/FRO has expired for this project. Therefore, this must be
submitted as a new permit application and should be submitted to the Central Office for consideration. Also, note that
effective tomorrow, October 3rd the permit application fees for the Division have increased due to recently approved
legislation. For flows less than 10,000 gpd, the new application fee for a pump'n haul permit is $1,160 instead of the
$810 listed on the attached information.

Dale, regarding the extension for the pump and haul, is there a particular form needed for an extension or should I just
prepare a letter? Is there a fee?

Kevi n,
A review of the application and supporting documents for the subject application has been completed in accordance
with 15A NCAC 02T .0107. The items listed below have been identified and prevent further action on this application
package until they are resolved. Please provide the additional or revised information for the following comments as
noted below, in order for the office to complete the review:
1. The subject project proposed 2-inch force main is designed to connect to the existing 12-inch sewer force main
owned by Fayetteville PWC at the DAK / Wellman Plant site. As noted in an e-mail to you dated June 8, 2022
from the Division's previous permit reviewer assigned to this project, Charlie Miller, the Division cannot issue a
permit for this subject project until all the issues associated with the existing FPWC 12-inch force main are
satisfactorily resolved. To date, they have not been resolved. The Division is now actively pursuing a solution to
this impasse, and I will keep you updated as to progress.
2. The pump and haul permit issued by the Division's Fayetteville Regional Office to your client for on -going
operations at the subject project site has expired as of March 13, 2023. Please submit a request to the Division's
Central Office to renew and extend this permit as soon as possible.
3. The permit application indicates a proposed flowrate of 2,500 gpd based on 100 employees per shift at 25 gpd
per employee. In the pump station design calculations submitted with supporting information there is a
reference to 2 shifts per day. Please clarify or revise the permit application.
4. The FTSE and the flow acceptance letter from FPWC included in the permit application package are both dated
November 9, 2021 and must be updated.
5. Please provide a detailed site plan for the proposed triplex pump station.
6. Please provide anti -flotation calculations for the proposed pump triplex pump station.
7. Please provide a copy of the construction specifications for the proposed project. The checklist enclosed in the
permit application package indicating portions of FPWC's standard construction specifications applicable to this
project is insufficient to meet the Division's requirements for construction specifications.
8. On the engineering plans sheets C1.1, C2.1.1 and C2.1.2 there are locations for the proposed 2-inch sewer force
that appear to be within 10 feet of the existing and proposed water mains. Please add notes to the engineering
plans that indicate a minimum horizontal separation of 10 feet will be maintained.
A response to the above comments is considered required in order to proceed with the review of the application and
issuance of the permit. If a complete response is not received within 30 days, the application and supporting documents
will be returned to you in accordance with 15A NCAC 02T.0107(e)(2).
Please feel free to provide digital copies of these revised documents and supporting information to me via email along
with hardcopies via mail.
