miles to NCSR 3208 (Pepper Ridge Rd) . Turn right, go
approximately 1. 1 miles to creek. Residence is the first on
the right past the creek.

and taken to the nearest municipal wastewater treatment
plant for disposal.

SUBJECT: NPDES Permit Application
Request for Permit Renewal
Permit Number NC0044202
Ms . Majora K. Gondring Residence
Forsyth County
Attached please find a staff report and recommendations
pertaining to the above mentioned project. The system has
been installed and is functioning adequately. This was
verified during the month of December 1987 . The only
addition to the original staff report would be to add the
Wastewater Code which is 04 for single family dwellings and
the stream classification of All is now referred to as
WSIII .
I recommend the permit be reissued and the applicant be
allowed to continue to discharge.
