Thank you for your earlier call, and my apologies for the belated reply. Since the property has changed hands, we’ll
need to get the permit transferred into your name. This will also waive any previous overdue fees from before the
permit was in your name, and ensure any future annual invoices are addressed to you.

Please see the attached Change of Ownership application. I’ve filled as much information as I could from our
database and County records; please double-check everything for accuracy and add any missing information. We
will need your signature on pages 2 and 3. You may send this form back to either me or Non.Discharge-
