Please upload the Certificate of Completion from Chatham County Watershed Protection Dept. It should
have the project name, Chatham Permit #, COC #, and the close out termination language.
NOT certification form needs to be sign with an original wet signature. No digital/computer generated
signatures are accepted.
On the NOT Certification Form, the Legally Responsible Person should be Thomas Quackenbush. In
order for Chad Knapik to sign the NOT Certification form and submit the eNOT, Chad needs to print their
name and title on the "Print Name and Title of Signed if Authorized" line of the NOT Certification Form.
