The forms can be found on our website here under the "Renew an Existing Individual Permit" heading.These forms are
only required for existing facilities renewing permit coverage. If this is a new facility that is applying for first time permit
coverage,then these items are not required.

The facility finally got the check cut and we have been going over all of the items for submission. I am not familiar
with the SPPP Certification Form and Supplemental Information Form. Are these included in the permit or are they
separate documents? Can you please send me a link if they are on your website.

The SWPPP itself does not need to be submitted, however, along with EPA Forms 1 and 2F, please make sure to submit

In that link,the $1,150 fee is for a minor wastewater permit.There looks to be several references for$1,500 permit fees,
but those are not for the NPDES Stormwater Program.
Please email me a copy of the application when you mail it in (this lets me know it's on the way and I can keep an eye
out for it or reach back out to you if I don't receive it). Only one hard copy(with wet ink signature) is necessary.

Maybe this is for the fees prior to October.That one said $1,150 if I read it right.Just wasn't exactly sure which one was
correct. We will make the check out for the $1200.00.
We have completed both forms and the table and have one set of data for each outfall, which entered into the
appropriate table.
The package is in the process of being reviewed and we will get it submitted once that is done. I believe you previously
said I can email it to you, but you still need a hard copy with the wet signature. Is that correct? Do you need more than
one copy?

Can you please send the link where you saw the$1,500 reference?The application fee for a minor individual industrial
stormwater permit is$1,200.You would send the entire packet (application and check)to my attention.A complete
application for new permit coverage must be submitted, which includes EPA Form 1, EPA Form 2F, and all supporting
documents (including Tables A, B, C, and D in Form 2F). If you have additional monitoring data, please send what you
have in a spreadsheet format.

Thanks Brianna. I did see that form, but there was another link that said the fee was$1,150 instead of$1,200. So do we

The attached table indicates that permit fees may have changed since October 2023. Can you tell me what the fee for a
Minor Industrial Permit is?Also, looking at the ePayments website, it does not look like we can pay the initial permit fee
