Description of existing facilities: a 1,500 gallon baffled septic tank with an effluent filter; two Advantex
AX20 treatment units; a 2,500 gallon recirculation tank with a recirculation pump; a 2,500 gallon
storage/dosing tank with an irrigation pump, and high-water audio/visual alarms; an ultraviolet (UV)
disinfection unit; a rain sensor; control panels; a 0.31 acre drip irrigation area; and all associated piping,
valves, controls, and appurtenances.

running continuously; although the pumps were still operating as designed. This has been communicated
to both the permittee and Aqwa, and the permittee is working with Aqwa to have this repaired.

V. ADDITIONAL REGIONAL STAFF REVIEW ITEMS
There was no fence around the sprayfield and the alarms on the pump tank were running continuously; although the pumps were still
operating as designed. This has been communicated to both the permittee and Aqwa, and the permittee is working with Aqwa to have
this repaired.
