This number is used to identify the specific point that is being sampled prior to waste being applied to the fields. Large operations have multiple PPIs. This number allows us to know where the samples are being taken. There is only one PPI for your permit so your PPI number is 001. As you can see from the email from Wanda Gerald, the November 2023 submission was received and rejected. The March 2024 submission was also received and rejected. I am inquiring now as to the reasons for the rejection. Your ORC should have received an email stating that the submission was rejected and the reasons for the rejection. The ORC is given the opportunity to correct the errors and resubmit. Apparently, this did not happen. It may help to explain how the Notices of Violation are generated. 1. The ORC submits the permit required documents (NCMR, MDAR-1, etc.) electronically. 2. The ORC receives a receipt confirming submission of documents. This document is not an approval of your forms, it is only a notice that the ORC successfully sent them. 3. The submitted documents are checked over by Wanda Gerald in our central office for completion of basic information such as permit number, name and signature of ORC, etc. 4. Once Wanda completes her check, the submission is either rejected or approved. A rejection/approval receipt is generated and sent to the ORC. 5. If rejected, Wanda sends a rejection notice back to the ORC with an explanation of the rejection. The ORC is given the opportunity to correct the errors and resubmit. 6. If approved, Wanda enters data from the submitted documents into BIMS (our internal DEQ-DWR database) and scans the documents into Laserfiche. 7. After a couple of months, Raleigh Regional Office staff (me) runs an audit report in BIMS that identifies potential violations. 8. In this case BIMS did not "see" a NDIM R report for November 2023. 9. Based on this audit report, I checked Laserfiche and confirmed that the November 2023 Forms were missing. 10. 1 generate the NOV, as missing/late reports is a permit violation. I hope this clarifies the process of how this NOV was generated. As soon as I find out the reasons or both the November 2023 and March 2024 rejections, I will let you know.
