5. Directions to Site: Take Hwy 150 South out of Winston-Salem
and turn left onto Hickory Tree Rd. Proceed 0. 3 miles and
then turn right onto Farmbrook Lane (SR 1600) . Proceed 0. 25
mile and turn right onto Marajo Court. The Tucker SFR is
the third house on the left before crossing the creek.

in a subdivision and is surrounded by other dwellings.

Creek.

downstream uses: The U.T. to Miller Creek flows through
several back yards before reaching Miller Creek. Quail
Run MHP (NC0070033 , 0. 017 MGD) discharges directly into
Miller Creek, just below it's confluence with the U.T.

existing) : The existing system consists of a 1000 gallon
septic tank and 600 feet of nitrification line followed by a
391 square foot subsurface sandfilter.

from the by septic tank local pumpers as necessary for

disposal in the nearest POTW.

PART IV - EVALUATION AND RECOMMENDATIONS
The nitrification line system for the Tucker residence was
constructed in 1977 and began failing shortly thereafter, in
January 1978 due to seasonal high water tables. To remedy the
problem, a sandfilter system was added on to the end of the
N-Lines in May 1979 . Observations of the effluent discharge on
6/16/92 indicates that the system is functioning properly. The
effluent pipe (4" PVC) discharges into the small stream adjacent
to the yard. The pipe outlet is approximately 21 feet north of
the back corner of the chain link fence.
Please note that no disin
fection facilities have ever been
constructed. The permit should contain a clause requiring
installation of a chlorinator within 180 days after reissuance.
