Dear Mr. Everett:
On May 26, 2011, Ms. Jenifer Carter of this office performed a General Permit Inspection on the
wastewater treatment system at the abovementioned address in Davidson County,North Carolina.
During this inspection, it was noted that chlorination disinfection and dechlorination have still not been
installed, as required in a Notice of Violation dated April 7, 2008. Authorization to Construct(A to C)
was issued for the construction of a chlorinator, chlorine contact tank, dechlorinator and a pump tank on
December 12, 2008. The A to C was issued based on plans received by our Construction Grants &Loans
Section on May 21, 2008 from John Beeson, PE, on your behalf.
This office requests that you respond to this letter, in writing, within 14 days of receipt of this Notice.
Construction, as per the approved plans, must be completed within 90 days of receipt of this Notice.
Your written response should include the following:
• Acknowledgment of and reasons for not constructing the required improvements as per the A to C.
• The name of a duly registered Professional Engineer in the State of North Carolina being used to
observe the construction of the chlorination/dechlorination system as per the approved plans and
specifications.
• If more than 90 days is needed to complete the construction, include reasons why, along with any
supporting documentation
A copy of the A to C, from the letter dated December 12, 2008, is enclosed. Please refer to it for
requirements related to construction of the required elements, as well as notification requirements.

Authorization to Construct Permit. In accordance with your application for discharge, the
Division is also authorizing the construction of a chlorinator and chlorine contact tank,
dechlorinator, and a pump tank into an unnamed tributary to Miller Creek, classified C NSW
waters in the Yadkin Pee-Dee River Basin. All elbow piping must be of the long sweeping type.
All cleanouts are to be housed in meter boxes below the surface. This system must be at least 50
feet from the dwelling and property lines and at least 100 feet from water supply wells on and off
the site. The system must also be constructed and located above a 100-year flood.
This Authorization to Construct permit is issued in accordance with Part III, Paragraph 2 of
NPDES Permit No. NCG550000, and shall be subject to revocation unless the wastewater
treatment facilities are constructed in accordance with the conditions and limitations specified in
Permit No. NCG550000. In the event that the facilities fail to perform satisfactorily, including
the creation of nuisance conditions, the Permittee shall take immediate corrective action,
including those as may be required by this Division, such as the construction-of additional or
replacement wastewater treatment or disposal facilities. Failure to abide by the requirements
contained in this Authorization to Construct may subject the Permittee to an enforcement action
by the Division of Water Quality in accordance with North Carolina General.Statute 143-21.5:6A
to 143-215.6C.
The Winston-Salem Regional Office, telephone number 336-771-5000, shall be notified at
least forty-eight (48) hours in advance of operation of the installed facilities so that an in-
place inspection can be made. Such notification to the regional supervisor shall be made
during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.
Upon completion of construction and prior to operation of this permitted facility, an Engineer's
Certification must be received certifying that the permitted facility has been installed in
accordance with the NPDES Permit, the Certificate of Coverage,this Authorization to Construct
and the approved plans and specifications. A leakage test shall be performed on the septic tank
and dosing tank to insure that any exfiltration occurs at a rate that does not exceed twenty (20)
gallons per twenty-four (24). hour per 1,000 gallons of tank capacity. The Engineer's
Certification will serve as proof of compliance with this.condition. Mail the completed
Engineer's Certification to the NPDES Unit, 1617 Mail Service Center, Raleigh, NC 27699-
1617.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for
the life of the facility.
