Andrews Properties proposes to construct a new maintenance building at the existing
Hampstead Chapel located at 17750 US Hwy 17 in Hampstead, NC. There is an existing
Low Density NCDENR State Stormwater Permit for the project site, SW8 940805, which
will be modified to account for the additional impervious surface area generated by the
proposed project improvements as well as impervious surface area constructed on the site
since the original permit was issued.
The project area is located within the White Oak River Basin and ultimately discharges to
Old Topsail Creek, Stream Index 18-87-12, which is classified SA; HQW.
The existing stormwater permit for the site has a permit area of 17.07 acres (743,730 SF)
and a permitted impervious surface area of 1.65 acres (71,686 SF), for a total percent
impervious of 9.64%. To facilitate the proposed and future improvements, the existing
stormwater permit will be modified to reflect the following:
• Maintain the permit boundary area of 17.07 acres
• Increase the impervious surface area from 71,686 SF (9.64%) to 223,119 SF (30%)
broken out as follows:
o On-Site Buildings/ Lots: 1,650 SF
o On-Site Streets: 6,366 SF
o On-Site Sidewalks: 625 SF
o Future: 125,956 SF
o Existing: 88,522 SF
Mortuary Site Drainage:
The area around the mortuary, which is where the new maintenance building will be
constructed, drains to a series of three interconnected existing wet detention ponds under
an innovative permit. No new stormwater conveyance systems are proposed in the area of
the maintenance building and runoff will be conveyed away from the building by sheet
flow.
Cemetery and Access Road Drainage:
The existing runoff from the cemetery and access road area is conveyed via a small existing
drainage collection system to an approximately 300 foot long grassed Swale approximately
10 feet in width. The stormwater collection system was included in the original permit and
was installed in order to properly conduct funeral services during unexpected rain storms
and collects runoff from both the grass and asphalt areas. No revisions are proposed to the
collection system with this permit modification. The permit modification will address
impervious surface areas for sidewalks and mausoleums in this area that have been
constructed since the permit was issued and provide an allocation for future improvements
in this area including an anticipated access road connection to Transfer Station Road.
In accordance with low density requirements, impervious surface area and stormwater
collection systems have been limited to only what is required to safely accommodate the
visitors and operations of the facility.

On June 10,2015,the Wilmington Regional Office of the Division of Energy, Mineral and Land Resources(DEMLR)
conducted a follow-up inspection of Andrews Mortuary Hampstead Chapel, located on Market Street in Ponder County to
determine the status of compliance with the State Stormwater Management Permit Number SW8 940805 issued on
November 28, 1994. DEMLR file review and site inspection revealed that the site is in compliance with the terms and
conditions of this permit. Please find a copy of the completed form entitled 'Compliance Inspection Report'attached to
this letter,which summarizes the findings of the recent inspection.
Please be advised that you are required to comply with the terms,conditions and limitations of the Stormwater
Management Permit under Title 15A North Carolina Administrative Code 2H.1003 and North Carolina General Statute
143-214.7,including operation and maintenance of the permitted stormwater system. Violations of your Stormwater
Management Permit may be subject to the assessment of civil penalties of up to$25,000 per day per violation.
If the property covered by this permit has been/will be sold or legally conveyed to another entity or if the permit holder has
changed its name and/or mailing address,or if the project name has been changed, it is your responsibility as the
permittee to submit a completed and signed Name/Ownership change form to DEMLR at least 30 calendar days prior to

Dear Ms. Lewis:
On December 16, 2014, Richard C. Andrews, Jr. received a Notice of Inspection letter from the
North Carolina Department of Environment and Natural Resources (NCDENR) indicating that an
inspection of the Andrews Mortuary — Hampstead Chapel site was not in compliance with the
terms and conditions of State Stormwater Management Permit Number SW8 940805. This letter is
to provide an update on the status of the actions to address the items identified in the Notice of
Inspection.
Deficiencies identified by NCDENR's Notice of Inspection are listed below in bold text with the
action taken and current status of the item in italics.

WK Dickson provided the Designer's Certification to NCDENR on February 18, 2015.
Please submit a permit modification to address the BUA that was added without benefit
of approval from the Division, and which includes the proposed new storage building.
WK Dickson held an express stormwater permit scoping meeting with NCDENR staff on May 14,
2015. An express stormwater permit submittal will be delivered to NCDENR on June 19, 2015 in
accordance with the submittal date established at that meeting.

t°L 11 �K WK Dickson coordinated with the Owner to have the required pond maintenance completed and
provided a letter to NCDENR on May 7, 2015 confirming that the required maintenance had been
completed.
In addition to addressing the deficiencies itemized in the Notice of Inspection letter, the required
documents to change the property owner name to Andrews Properties, Inc. were submitted to
NCDENR on March 23, 2015. NCDENR issued a letter on May 7, 2015 indicating that the name
change had been approved by NCDENR.

PRELIMINARY PROJECT NARRATIVE

Andrews Properties proposes to construct a new maintenance building at the existing
Hampstead Chapel located at 17750 US Hwy 17 in Hampstead, NC. There is an existing
Low Density NCDENR State Stormwater Permit for the project site, SW8 940805, which
will be modified to account for the additional impervious surface area generated by the
proposed project improvements as well as impervious surface area constructed on the site
since the original permit was issued.
The project area is located within the White Oak River Basin and ultimately discharges to
Old Topsail Creek, Stream Index 18-87-12, which is classified SA; HQW.
The existing stormwater permit for the site has a permit area of 17.07 acres (743,730 SF)
and a permitted impervious surface area of 1.65 acres (71,686 SF), for a total percent
impervious of 9.64%. To facilitate the proposed and future improvements, the existing
stormwater permit will be modified to reflect the following:
• Maintain the permit boundary area of 17.07 acres
• Increase the impervious surface area from 71,686 SF to 223,119 SF broken out as
follows:
o On-Site Buildings/ Lots: 1,650 SF
o On-Site Streets: 0 SF
o On-Site Sidewalks: 625 SF
o Future: 132,322 SF
o Existing: 88,522 SF
• Increase the total percent impervious from 9.64% to 30%
Mortuary Site Drainage:
The area around the mortuary, which is where the new maintenance building will be
constructed, drains to a series of three interconnected existing wet detention ponds under
an innovative permit. A small roof drainage system will convey the runoff from the new
maintenance building to the third of the three stormwater ponds. The runoff from the
concrete slab and gravel areas associated with the maintenance building will drain by sheet
flow into the same pond.
Cemetery and Access Road Drainage:
The existing runoff from the cemetery and access road area is conveyed via a small existing
drainage collection system to an approximately 300 foot long grassed swale approximately
10 feet in width. The stormwater collection system is needed in order to properly conduct
funeral services during unexpected rain storms and collects runoff from both the grass and
asphalt areas. No revisions are proposed to the collection system with this permit
modification. The permit modification will address impervious surface areas for sidewalks
and mausoleums in this area that have been constructed since the permit was issued and
provide an allocation for future improvements in this area including an anticipated access
road connection to Transfer Station Road.
In accordance with low density requirements, impervious surface area and stormwater
collection systems have been limited to only what is required to safely accommodate the
visitors and operations of the facility. Runoff from the site impervious surface is primarily
conveyed to vegetated swales or the existing stormwater ponds by sheet flow.

I finally got up with Linda about this project.
1. Can you please email me the PE Cert for this one? It has evidently been misplaced.
2. Her comments in her inspection regarding pond slopes had to do with maintenance/erosion and undercutting at
the bottom. From your letter, it sounds like that has been resolved. We will get the inspector to re-inspect to
close that loop.
I have drafted the permit. As we discussed, it is in a different format because the COC from 1994 isn't available
electronically. I basically took the points in that letter, and retyped them into an itemized list the way our permits
currently look now. Draft attached. I converted some data from acres to square feet, so I will clean that up when I get
your package next month if need be.

I may have changed ownership from "Unknown WiRO" to whatever was on the permit, but I would have done that on
BIMS. The only other change was from one LLC to another, where the principal was the same. At the time, I thought that
we could change the name of the company without a signed form, but I am aware now that even if it's the same
principal, it's still an ownership change. I was probably going to do it all at one time, but I don't remember. I print out a
lot of ownership information, but until the signed form is in hand, I don't usually change ownership in BIMS. There's no
point in addressing a letter to a company that doesn't exist or to a company that is not the property owner.
Did you have any concerns about what was discussed at your meeting?#2 doesn't have any questions associated with it.
I don't see anything in my piles that looks like a certification, but I do remember getting one. Whatever I received went
into the file. If it's not there, then I don't know what happened to it-sorry.
If the steepness of the slopes was mentioned in the inspection report, then I must have seen something in the way of
erosion. I remember the slope at the water's edge being undercut instead of sloped. We allow existing borrow pits to be
used as ponds with steeper slopes as long as the slopes are maintained and vegetated to avoid erosion and
undercutting.

Four hopefully simple questions:
1. It looks like you issued an ownership change on 5/7/15 for this, but it doesn't look like BIMS got updated. I was
going to change it, but then Georgette said that there may have been a correction and so I didn't want to screw
something up.
2. They came in this morning to talk about a modification to fix your inspection issues from 12/10/14
3. They also said they just sent in a letter with the certification and information on the maintenance that has been
completed. Do you have that letter in your pile? I don't see it in the folder.
4. The inspection also talks about the slopes of the pond being steep. These were old borrow pits and it looks like
they were permitted in 1994 as-is. Is there something I need to look for in the maintenance to address this
issue, or was that just a note in the inspection report that didn't require formal changes onsite?
After they mod the permit to reflect the BUA that has been built, this should get everything resolved for this one.
