Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of
this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this pemvt is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the Division of Land
Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be
required.

1. Upstream = at least 100 feet upstream from the outfall. Downstream = at least 100 feet downstream from
the outfall. Instream monitoring requirements are waived as long as permittee retains membership and
participation in the Middle Cape Fear River Basin Association.
2. The discharge from this facility shall not cause turbidity in the receiving stream to exceed 50 NI'U. If the
instream turbidity exceeds 50 NI'U due to natural background conditions, the discharge cannot cause
turbidity to increase in the receiving stream.
All samples collected should be from a representative discharge event.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

I am writing in response to your letter of September 12`h, 2001, in which you enclosed a
draft NPDES Permit (NC0076783) for the PO Hoffer Water Treatment Plant. In the
letter you requested that we review the draft permit and submit comments within 30 days
following receipt of that letter.
The only comment we would like to make is in Section A. (1.) regarding monitoring
requirements. The draft permit contains a requirement to conduct weekly upstream and
downstream monitoring for turbidity. I would like to request that this requirement be
deleted in lieu of the fact that the PO Hoffer WTP is a member in good standing of the
Middle Cape Fear River Basin Association. The Public Works Commission has already
signed a Memorandum of Agreement with NCDWQ that waives in -stream monitoring for
this facility as long as the permittee is a member of the Association. I have attached the
letter from NCDWQ waiving this requiement.

Applicant is not requesting modification of facility or increasing flow at this time.
A rating sheet is not attached as no modifications were made to this facility during the previous
permit cycle.
A review of compliance data did not reveal any significant or repetitive violations.
Based on the last NPDES Compliance Inspection, there are no outstanding repairs or
modifications of the treatment works necessary at this time.
No special conditions, limitations, or monitoring is suggested at this time.
Based on the above information, the Fayetteville Regional Office recommends reissuance in

As part of the inspection, a tour of the Water Treatment Plant was conducted. All
observations and recommendations are in Part D. (Summary of Findings/Comments) of this
inspection report. Remember, a good effluent is essential to insure the quality of the surface waters
of this State.

Section D: Summary of Findings/Comments:
1. A copy of the NPDES permit was available upon request.
2. The Compliance Evaluation Analysis Report for the period of January, 1999 through December, 2000
was reviewed. One violation occurred in May, 2000 when Total Suspended Solids exceeded the
monthly average discharge limit of 30.0 mg/l. The average for that month was 43.0 mg/I.
3. Laboratory data was reviewed for the months of November, 2000 and December, 2000. All data
seemed to be correct as reported on the DMR's, except for Residual Chlorine data for the month of
November. Laboratory results are analyzed as milligrams per liter, but data has to be reported on the
DMR sheets as micrograms per liter. There was an mathematical error in converting from mg/1 to ug/1
on all of that months chlorine results. After talking with Mr. Jeff Carlisle, "Chief Operator," a new
amended DMR report will be sent to this office explaining the errors.
4. The required parameters Iron, Aluminum, and Total Suspended Solids are analyzed at the PWC
Cross Creek WWTP. Turbidity, pH, Residual Chlorine, and Settleable Solids are analyzed on site.
5. Flow rate is calculated by measuring the height of the water coming out of the discharge pipe and
the diameter of the discharge pipe and converting to MGD. Maintenance records are also being
properly documented and maintained.
6. The wastewater at this facility is solids from the sedimentation basins and backwash water from the
filter system. The Glenville WTP also discharges all of its wastewater into the Hoffer WTF. The
wastewater from both of these facilities is pumped into an equalization basin. A polymer is added
and then pumped into two (2) clarifiers. Solids settle out and the clear water overflows the weirs and
is gravity fed to a ten (10) acre lagoon. Clear water from the lagoon is then discharged to the Cape
Fear River. Sludge is currently being removed from the lagoon by a contractor.
7. . The outfall was easily accessible and the effluent appeared to be very clear.

Enclosed are three copies of an application for renewal of the subject NPDES
permit. We understand through contacting your office that a $400 processing fee for the
facility is no longer required.
The discharger is the P.O. Hoffer Water Treatment Facility. The wastewater to be
discharged is supernatant from a settling lagoon fpr alum sludge and filter backwash
water. Tests indicate compliance with the Administrative Code Section 15 NCAC 2H
.0130 "General Permit for Water Filtration Facility".

The Division is beginning the cycle of permit renewals for the Cape Fear river basin. An examination of the Cape
Fear basin plan and the NPDES permit schedule has revealed that the existing expiration dates in Cape Fear NPDES
permits do not coincide with the revised permit expiration dates for the basin. This discrepancy could create unnecessary
delays in the processing of over 200 permit renewals. Accordingly, the Division is changing the permit expiration dates for
NPDES permits in the Cape Fear river basin.
This permit modification changes the expiration date of the subject permit in subbasin 30615 to October 31, 2001.
Please find enclosed the revised permit cover page. Insert the new cover page into your permit and discard the old page.
All other terms and conditions contained in the original permit remain unchanged and in full effect. This permit
modification is issued under the requirements of North Carolina General Statutes 143-215.1 and the Memorandum of
Agreement between North Carolina and the U. S. Environmental Protection Agency.
In accordance with the modification of the permit expiration date, the new due date for your renewal application is
May 4, 2001. Renewal application forms will be sent to you approximately 6-8 weeks in advance of the new due date or
you may download the forms from the NPDES web site at http://h2o.enr.state.nc.us/NPDES/documcnts.html. You
will need to download Short Form C. If you have already submitted a permit renewal form, there is no need to submit
any other forms to the Division; the application will be processed during the renewal period for your subbasin.
