A Compliance Evaluation Inspection was performed on January 28, 2016 by George Smith. The inspection is
comprised of an overall facilities site evaluation and to determine if the wastewater treatment plant was
constructed.
Permit
Your NPDES permit became effective on October 1, 2012 and expires on September 30, 2017.
Facility Site Review
There is no facility at the site. The location of the site is off Shulls Mill Road on the right, at the end of
Labrador Run (approximately 0.5 miles from a road on the left named Azelea).
The compliance evaluation inspection is satisfactory. Please remember that you must apply for and receive an
Authorization to Construct before you proceed with a wastewater treatment design. If you have any questions
