Dear Mr. Freeman:
The Division of Water Quality (the Division) hereby corrects an error in your recently issued permit,
effective September 1, 2008. After recent correspondence with Mr. David Hemenway, your Operator
in Responsible Charge (ORC), we have revised your facility map for the subject site to reflect the
correct outfall location.
Attached, please find a permit change page. Please insert this page into your existing permit and
discard the old page.
This permit is not transferable except after notice to the Division of Water Quality. The Division may
modify, or revoke and reissue this permit. This permit does not affect your legal requirements to obtain
other permits required by the Division of Water Quality, the Division of Land Resources, the Coastal
Area Management Act, or any other Federal or local government.
