Division of Water Resources' Central and Regional staff has reviewed the application package
received on November 1, 2023, and additional information received on March 7, 2024. However, the
Division requires additional information before completing our review. Please address the items on the
attached pages no later than the close of business on June 21, 2024.
Please be aware that the Applicant is responsible for meeting all requirements set forth in North
Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the
review of the subject application package. The Division may return the application as incomplete pursuant
to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in
Sections A through O or the Applicant fails to provide the additional information on or before the above -
requested date.

The response to Comment D.2 of my previous additional information request stated that the
proposed overflow/by-pass line is a part of the "approved proprietary system" but doesn't clarify
what line it is referring to. The prohibition of by-pass or overflow lines is to prevent untreated
wastewater from being sent through the treatment system. The recirculation line may be confused
as a "by-pass", but it is designed to only send treated wastewater through to the next unit process.
Please clarify whether the by-pass line that is provided in response to Item V.5 of the SFRWWIS
06-16 is this recirculation line.
2. In Item IV.19. of the SFRWWIS 06-16 form, the "Public right of way" setback and the "building
foundation" setback are still listed as "Wavier". The documentation provided did not provide a
waiver of these setbacks. Please update this table with the minimum field observed distance from
the irrigation system.
