NCG140000 on behalf of Raleigh Concrete LLC to modify the existing Certificate of Coverage NCG140463 for the
Raleigh Concrete—Raleigh Plant at 1938 South Wilmington Street, Raleigh, NC 27603 to include the associated
Vehicle Maintenance Shop located adjacent and to the north at 95 Bluff Street. This modification is being
submitted in response to the recent Notice of Violation (NOV) 20223-PC-0579, dated March 21, 2024, and
guidance provided by Mr. Thad Valentine of the DEMLR during a recent site meeting on April 23, 2024. The
following NOI application form and supporting documentation are enclosed:

Receiving water is Walnut Creek within the Neuse River Basin[Stream Index 27.34-(4)a],designated as Class C;NSW waters. This segment of Walnut Creek is listed in the
2022 Integrated Report 303(d)list of impaired waters as exceeding criteria for Fish Community(Nar,AL, FW)and PCB Fish Tissue Advisory(Advisory,FC, NC). TMDLs
have not been established specific to Walnut Creek. There is a state-wide TMDL for mercury,which does not impact monitoring for Raleigh Concrete.
