You are listing both Thomas Fits and Fits and Goodwin, Inc as the project's responsible entity/permittee. Only one
should be listed. The correct responsible entity/permittee should be listed on the FRO form, which should then be sent
to the Washington Regional Office to ensure they are addressing the correct responsible entity/permittee on their letter
of approval. That same responsible entity/permittee should be listed on the NOI certification form (uploaded to Tab D9),
as well as Tab B1, and Tab E.
Please submit a new NOL To auto -populate the new NOI, select'Yes' for the first question & enter NOI number 196137.
