On October 29, 2007, the NC Division of Water Quality (DWQ) received the application for an Individua1401 Water Quality
Certification for impacts associated with the above referenced project. An Individua1404 Permit will be required for this project. Please
note that the following must be received prior to issuance of a 401 Water Quality Certification:
1. The PCN Application failed to contain the Agent's Signature and Date. See last page of application. An agent authorization
form was included with the application, however, the application itself was not signed. Please supply one (1) original and
four (4) copies of the signature page. Action is required on your part for this omission.
The Public Notice has been received by the USACE. Until the information requested in this letter is provided, I will request (by
copy of this letter) that the Corps of Engineers place this project on hold. Also, this project will be placed on hold for our processing due
to incomplete information (15A NCAC 2H .0507(h)).
