Please find enclosed a submittal package for the proposed installation of storm water
facilities for the subject project. The project generally consists of building additions and renovations
to the existing Blackwell Hall building on the campus of Mars Hill University, including demolition
of an existing structure and its associated parking, and removal and reconfiguration of hardscape
areas around and directly adjacent to Balckwell Hall. Communication has been had with Mr. Stan
Aiken, PE, regarding this submittal and the concurrent Erosion Control submittal made for this
project. The Express Option for the erosion control submittal was selected. Mr. Aiken indicated
that approval of the erosion control submittal could occur prior to issuance of storm water approval
with the understanding that the submittal for the storm water permit must have been made prior to
approval of the erosion control submittal. The erosion control submittal was made on March 14th,
via the online submittal portal, and was assigned a number of PA-003820. Included with this
submittal package are the following items for your review:
• One (1) original and one (1) copy of the Application.
• Original Supplement Forms and O&M. Please note that the EZ Supplement forms have
been signed and sealed, the O&M Agreement has been included as a draft, for review prior
to execution. When reviewed, if found satisfactory, the document will be executed and
returned to your office.
• Completed Notification of Drainage Well Form. Please note that this form has been
included as a draft, for review prior to execution. When reviewed, if found satisfactory, the
document will be executed and returned to your office.
• Check in the amount of$505 for the application fee.
• A narrative,included in the calculation manual.
• A USGS map indicating the project location.
• Calculation manual.
• Two sets of plans.
• Property deed information. Please note that Madison County Register of Deeds indicates a
deed book and page of "0". A copy of information taken from the Madison County GIS
system as well as the Madison County Register of Deeds is included with this submittal.
Madison County Register of Deeds indicates a deed date of 1/1/1900, but does not provide
deed book and page, nor plat book and page information.
