Dear Mr. Phthisic:
In accordance your request for an NPDES permit renewal received on September 18, 1997, the Division is
forwarding herewith the subject NPDES permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated December 6, 1983.
The following changes (which originally appeared in the draft permit) appear in the final permit:
Monitoring requirements for Total Nitrogen and Total Phosphorus have been included in the permit
renewal. Special conditions A.(2) and A.(3) have been added to the permit renewal to guide you
through the revised monitoring requirements and load calculations. This action is a result of the rule
supporting the Neuse River Basin Nutrient Sensitive Waters Management Strategy, recently adopted
by the Environmental Management Commission.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days
following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer
27447. Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and
binding.
Please take notice that this permit is not transferable except after notice to the Division of Water Quality.
Contact Division personnel in case of any change in ownership or control of this discharge. The Division of Water
Quality may require modification or revocation and reissuance of the permit.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of land Resources, the Coastal Area Management
Act or any other Federal or Local governmental permit that may be required. If you have any questions

forth in Parts I, II, III, and IV hereof.
The permit shall become effective August 1, 1999.
This permit and the authorization to discharge shall expire at midnight on May 31, 2003

A.(2). Total Nitrogen Monitoring
The Permittee shall calculate annual total nitrogen mass loading as the sum of monthly loadings,
according to the following equations:

where:
Days = The number of discharge days in that month
TN = The average total nitrogen concentration (mg/1) of the composite samples
collected during the month
QW = The average daily waste flow (MGD) for the month
8.34 = Conversion factor, from (mg/l x MGD) to pounds

The annual mass loading is the sum of the monthly loadings:

Summary
Facility has requested a permit renewal with no modifications to the existing facility
operations.
A review of the most recent compliance evaluation inspection report and all available DMR
data was performed. The most recent compliance inspection report was prepared on 12-
18-97. At that time, the facility appeared to be well maintained and the effluent was free
of solids. The DMR review revealed pH violations during the October 1996 reporting
period. No other violations were noted. One inconsistency was noted during the DMR
review. It is unclear where the facility is performing instream sampling. The facility
inconsistently reports the upstream and downstream location and sometimes does not
report a location at all. Region please comment on this issue.

Z he discharge shall not cause the turbidity of the receiving water to exceed 50 NTU. If the turbidity exceeds these levels due to natural
background conditions, the discharge level cannot cause any increase in the turbidity in the receiving water.
All samples collected should be of a representative discharge.
3 The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units and shall be monitored weekly at the effluent by grab
sample.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

Enclosed is the following for renewal of the NPDES permit for the Alum Sludge
Handling Facility at the Smithfield Water Treatment Plant.
Three (3) copies of the application.
Check in the amount of $200 for permit renewal.
Sludge Management Plan
The discharge pennit is for the decant water from the alum sludge treatment facility
at the Smithfield Water Treatment Plant. The alum sludge will be dried on a vacuum
drum and the sludge will be disposed of in a landfill.
