Summary Facility has requested a permit renewal with no modifications to the existing facility operations. A review of the most recent compliance evaluation inspection report and all available DMR data was performed. The most recent compliance inspection report was prepared on 12- 18-97. At that time, the facility appeared to be well maintained and the effluent was free of solids. The DMR review revealed pH violations during the October 1996 reporting period. No other violations were noted. One inconsistency was noted during the DMR review. It is unclear where the facility is performing instream sampling. The facility inconsistently reports the upstream and downstream location and sometimes does not report a location at all. Region please comment on this issue.
