REQUEST FOR MORE INFORMATION
Dear Mr. Etowski:
On August 28, 2007, the Division of Water Quality (DWQ) received your application dated August 23,
2007, to fill or otherwise impact 0.021 acres (actually adds up to 0.088 acres) of 404/wetland and 58
linear feet of perennial stream to construct the proposed residential development at the site. On October
17, 2007, the DWQ received additional information from you, however the DWQ has determined that
your application remains incomplete and/or provided inaccurate information as discussed below. The
DWQ will require additional information in order to process your application to impact protected
wetlands and/or streams on the subject property. Therefore, unless we receive five copies of the
additional information requested below, we will have to move toward denial of your application as
required by 15A NCAC 2H .0506 and will place this project on hold as incomplete until. we receive this
additional information. Please provide the following information so that we may continue to review your
project.
Additional Information Requested:
As previously requested, please provide the following information in hard copy. via mail or FedEx:
a. The location of the entire site boundary (all phases) on the USGS topographic map and
Harnett County Soil Survey.
b. The complete site plans on full plan sheets with elevation contours at a scale of no
smaller than 1 inch 3 50 feet.
c. Please provide conceptual lot layout, road layout and all impacts for all phases of the
project.
2. The letter received by the DWQ on October 17, 2007 from your consultant, Thomas S. Blue,
indicates that ten bioretention areas are proposed to treat the stormwater from this site. For each
proposed bioretention area, please provide one completed Bioretention Supplement Form,
available on our web site at: http://h2o.enr.state.nc.us/su/bmp forms.htm. You must fill out both
worksheets of each form and provide all items listed in the Required Items Checklist.

and you will need to reapply with a new application and a new fee.
