On April 17, 2024, Jim Westcott from the Raleigh Regional Office visited your wastewater
treatment lagoon and spray irrigation system to evaluate compliance with the subject permit.
The assistance of Tony Creson and Matt Hieshetter at the time of the inspection was
appreciated.
The facility is permitted for the operation of a 4,000-gallon recirculation tank with two 480
gallon per minute (GPM), 0.5 horsepower (hp) recirculation pumps; an AdvanTex AX-100 textile
filter treatment system; and all appurtenances, and the continued operation of an 8,000 gallon
baffled septic tank with effluent filter; a 6,000 gallon recirculating pump tank (modified to
drain into the new 4,000 gallon recirculation tank); chlorine disinfection tanks; a 340,750
gallon storage pond; a 4,000 gallon irrigation pump tank with two 52 GPM pumps, and high
water alarms, a portable generator; a 2.5 acre spray irrigation area; and all associated piping,
valves, controls, and appurtenances with no discharge of wastes to surface waters.
The permit, which was issued November 08, 2019, is effective through September 30, 2024.
Division records indicate the required annual fee for the period ending April 30, 2023, due May
11,2024, is currently open.
Inspection criteria consisted of the following:
❑ Review of permit conditions.
❑ Evaluation of facility information and wastewater collection, treatment, and lagoon system.
❑ Review of logbook, operation, and maintenance plan (O&M), calibration records and Non -
Discharge Monitoring Report (NDMR) and Non -Discharge Application Report (NDAR-1).
❑ On -site inspection of wastewater lagoon treatment system and distribution components.
Findings during the inspection were as follows:
The wastewater generated from the Louisiana-Pacific OSB facility enters a lift station where
flow rates are recorded from an inline mag meter daily and averaged monthly. Flow is
transferred to an 8,000-gallon baffled septic tank for solids separation prior to entering the
AdvanTex treatment system.

The spray irrigation system consists of three zones and was evaluated for operation at the time
of inspection. All spray heads were discharging at an even flow rate and spray pattern.
The perimeter fencing surrounding the facility spray fields and a gate restricts access to the
storage lagoon and AdvanTex treatment unit. All contact tank(s) lids were secured. Proper
signage was located on the perimeter fencing.
The freeboard level in the lagoon was 4'. lagoon freeboard levels are measured weekly. Permit
required pH and Chlorine residual are analyzed per event.
The daily operations log and daily facility checkoff form(s) were complete. Pump run times,
media pad evaluation and condition of facility are recorded. Facility grounds were clean and
structured.
An O&M manual and facility logbook was available for review at the time of inspection.
Chlorine tablets were evident in the chlorination tubes at the time of inspection. The correct
supply of tablets was available on site. Disinfection systems use approximately 15-20 tablets
per week.
Residuals are removed from the system by McFarland Septic services as needed and are
transferred to the North Durham Water Reclamation Facility for processing.
Permit required lab analysis is conducted by Euro fin Laboratories. Effluent sample analysis
was available at the time of inspection.
The Annual Standard Soils fertility analysis is conducted by the North Carolina Department of
Agriculture and Consumer Services (NCDA & CS) and samples were last collected on November
27, 2023. Report of analysis was received February 21, 2024. Records were available for review.
A 13 kW Generac Guardian propane generator is verified operational weekly by engaging a local
disconnect.
No deficiencies were noted in the NDMR documents.
Please continue to periodically inspect and evaluate the wastewater spray irrigation system
components are properly maintained and operating as designed. Maintain all associated
operational records onsite for a period of fives as required by the subject permit.
