Division personnel have reviewed and approved your application for renewal of the subject permit
Accordingly, we are forwarding the attached NPDES discharge pexrnit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This final permit includes no major changes from the draft permit sent to you on August 30, 2006.
Limits for ammonia have been placed in your petmit This is a result of U.S. Environmental Protection
Agency requirements to develop daily maximum restrictions for every non -municipal permit that has a monthly
average limit Limits for enterococci have been placed in your permit The permittee shall monitor for
enterococci (with no effluent limit) for twelve (12) months following the effective date of this permit Limit
becomes effective December 1, 2007.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable
to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt
of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh,
North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit This permit does not affect the legal requirements
to obtain other permits which may be required by the Division of Water Quality or permits required by the
Division of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit

This permit and authorization to discharge shall expire at midnight on November 30, 2011.

collected three times per week from June through September, and once per week from October through May.

A. (3.) Phased Construction Requirement
Expansion above 0.4 MGD cannot begin until the average discharge flow exceeds 0.32 MGD. At no time
may the flow tributary to the WWTP exceed the existing design capacity of the WWTP.
A. (4.) PERMIT RE -OPENER: TMDL IMPLEMENTATION
The Division may, upon written notification to the. Permittee, re -open this Permit in order to incorporate or modify
effluent limitations, monitoring and reporting requirements, and other permit conditions when it deems such action
is necessary to implement TNDL(s) approved by the USEPA.

on the west side of the Brunswick River. Go under the Hwy. 74-76 overpass and take a right
onto NCSR 1551 Blackwell Road, travel 0.3 mi. and turn left onto NCSR 1524 Chappell
Loop Road and travel 1.2 mi.. The entrance to the W WTP will be on the left behind a locked
gate. The Brunswick County trash transfer station is adjacent to the site.

PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS
1. a. Volume of wastewater to be permitted: 0.4 MGD
b. What is the current permitted flow limit? •0.4 MGD, Note: The current permit states
that when the average discharge flow exceeds 0.32 MGD this facility can begin
expansion to 0.8 MGD. This office does not support this flow requirement to be met
prior to expansion of the facility, especially in light of the rapid development in the
areas serviced by this WWTP.
C. Actual treatment capacity of the current facility? 0.4 MGD is listed but the staff has
indicated that above 0.30 MGD rate, operational problems are encountered and the
tertiary filters become overloaded hydraulically, which often results in the need to
bypass these units.
d. Date(s) and construction activities allowed by previous Authorizations to Construct
issued in the previous two years: None.
e. Please provide a description of existing or substantially constructed wastewater
treatment facilities: Existing facilities consist of. influent bar screen, aerated flow
equalization basin and pumps, extended aeration tank with 3 blowers, clarifier, mixed
media tertiary filters, gas chlorine disinfection, post aeration section, aerobic sludge
digester, lime slurry addition system, stand by power generator, influent and effluent
sampling equipment and effluent flow measuring and recording equipment.

Two nitrogen ammonia exceedances in January, 2006 and July, 2003.
Three pH violations in February and March, 2003 and October, 2004.

PART IV - EVALUATION AND RECOMMENDATIONS
This facility generally produces a quality effluent within permit limitations, but over the last
year has experienced periods of high inflow from storm events. At times they have had to
bypass tertiary filters and in one case were required to bypass a portion of the equalization
basin (primarily rainwater) directly to the effluent to prevent overflow of the tank. The area
served by this facility is experiencing significant development, which is believed to be the
source of these excessive flows via leaks and lines opened during construction activities.
Also this facility has recently been experiencing problems with nitrification rates resulting in
effluent above the permitted limit for NH3, some of this problem can be attributed to the
excessive hydraulic loading mentioned above. Another problem has been related to solids
management at the facility. The WWTP at times has been forced to aggressively waste
sludge from the system and in doing so, has lost or significantly decreased the effective
nitrifying population. Even though the facility is permitted at 0.4 MGD, treating wastewater
flows over 0.30 MGD becomes troublesome for the operations staff. The North Brunswick
Sanitary District has expressed interest in an expansion project for this facility but any details
and plans regarding this project have yet to be formally submitted. Another project is in
development for this WWTP to have the capability to transfer untreated wastewater as
needed, from an onsite pumping station to the Northeast Brunswick Regional WWTP
(NC0086819) for treatment and disposal. The force main for this project has been completed
but the required pumping stations have not yet been constructed, the permit covering this
project is WQ0024594.
It is recommended the permit be reissued to the North Brunswick Sanitary District in
accordance with the CFR Basinwide plan for the Belville WWTP (NC0075540).

PLANT SLUDGE MANAGEMENT PLAN
1) All sludge is to be dewatered by settling 4aid decanting the water to the EQ basin.
2) After decanting the remaining sludge shall have hydrate lime added and mixed
until a PH of 12.8 throughout is reached and must remain above 12 for 24 hours.
3) After a continuous 24 hour period of the sludge remaining above 12PH the sludge
is then to be removed from the digester by S&B Maintenance (contract hauler and
disposal facility). They haul to their disposal fields and broadcast according to their
permit.
4) The digester is to be checked, including the pumps and aeration facilities, washed
down and returned to service.

APPLICATION OVERVIEW
Form 2A has been developed In a modular format and consists of a `Basic Application Information" packet
and a "Supplemental Application Information" packet. The Basic Application Information packet is divided
into two parts. All applicants must complete Parts A and C. Applicants with a design flow greater than or
equal to 0.1 mgd must also complete Part B. Some applicants must also complete the Supplemental
Application Information packet. The following items explain which parts of Form 2A you must complete.
BASIC APPLICATION INFORMATION:
A. Basic Application Information for all Applicants. All applicants must complete questions A.1 through A.8. A treatment works
that discharges effluent to surface waters of the United States must also answer questions A.9 through A.12.
B. Additional Application Information for Applicants with a Design Flow a 0A mgd. All treatm@ntWorks that have design flows
greater than or equal to 0.1 million gallons per day must complete questions 13.1 through B.6,
C. Certification. All applicants must complete Part C (Certification).

D. Expanded Effluent Testing Data. A treatment works that discharges effluent to surface waters of the United States and meets
one or more of the following criteria must complete Part D (Expanded Effluent Testing Data):
1. Has a design flow rate greater than or equal to 1mgd,
2. Is required to have a pretreatment program (or has one in place), or\%
3. Is otherwise required by the permitting authority to provide the information.
E. Toxicity Testing Data. A treatment works that meets one or more of the following criteria must complete Part E (Toxicity Testing
Data):
1. Has a design flow rate greater than or equal to 1 mgd,
2. Is required to have a pretreatment program (or has one in place), or
3. Is otherwise required by the permitting authority to submit results of toxicity testing.
F. Industrial User Discharges and RCRA/CERCLA Wastes. A treatment works that accepts process wastewater from any
significant industrial users (SIUs) or receives RCRA or CERCLA wastes must complete Part F (Industrial User Discharges
and RCRA/CERCLA Wastes). SIUs are defined as:
1. All industrial users subject to Categorical Pretreatment Standards under 40 Code of Federal Regulations (CFR) 403.6 and
40 CFR Chapter I, Subchapter N (see instructions); and
2. Any other industrial user that:
a. Discharges an average of 25,000 gallons per day or more of process wastewater to the treatment works (with certain
exclusions); or
b. Contributes a process wastestream that makes up 5 percent or more of the average dry weather hydraulic or organic
capacity of the treatment plant; or
C. Is designated as an SIU by the control authority.
G. Combined Sewer Systems. A treatment works that has a combined sewer system must complete Part G (Combined Sewer
Systems).

Cape Fear River Basin (Wilmington Regional Office)
On 12/13/2005, NPDES staff (Gil Vinzani, Maureen Crawford, Tom Belnick) met with Wilmington
Region Surface Water Protection staff (Ed Beck, Linda Willis), to consider facility compliance in
drafting NPDES permit renewals for facilities located in the Cape Fear River Basin. A short list of 20
facilities were identified within the WIRO which exceeded subjective compliance thresholds, and the
need for possible permitting actions for these facilities were discussed. A summary of these discussions
and recommended actions are presented below, and included with each permit file. Assigned permit
writers should incorporate recommended actions or conduct followup with WIRO and/or NPDES staff
as necessary. Please see NPDES staff listed above with any questions.
1) NC0002305 Guilford Mills East; permit unassigned)- Facility was under SOC and actions
completed. No action necessary. This permit does not expire until 12/31/2007, thus it was not assigned
with the Cape Fear 2006 permit renewals. The current expiration date is not consistent with the Cape
Fear River Basin schedule, so the next renewal should set the expiration date to 1/31/2012 to coincide
with the Cape Fear subbasin 030622 expiration schedule.
2) NC0002933 (Calypso WTP: permit assigned to Carolyn Brvant)- No action necessary.
3) NC0003344 (House of Raeford: permit assigned to Joe Corgoron)- Formerly Circle S poultry
operation, now under new ownership. WWTP lacks duality. Permit writer should contact WIRO (Linda
Willis) to discuss whether a special condition should be added to permit renewal.
4) NCO020346 (Magnolia WWTP: permit assigned to Maureen Crawford)- Facility has made
improvements to headworks. No action necessary.

5) NCO021334 (Southport WWTP; permit assigned to Maureen Crawford)- Facility has had problems,
due to historical innovative technology that never worked well. Facility is in process of taking discharge
offline, and diverting to SE Brunswick for land application. Contracts are in place. No action
necessary- but might want to add language to cover letter summarizing our understanding that facility
will ultimately eliminate discharge.
6) NCO023256 (Carolina Beach WWTP; permit assigned to Bob Sledge,)- Upgraded plant to 5&2 with
tertiary filters and small spray/reuse component. No action necessary.
7) NCO025763 (Kure Beach WWTP; permit assigned to Francic Candelaria)- Problem with TSS/algae
growth, which has been abated by aquashade and barley hay bales. Effluent currently looks good. No
action recommended.
8) NCO025895 (Holly Ridge WWTP; permit assigned to Maureen Crawford)- Recission request
received by DWQ- do NOT renew permit.
9) NCO026018 (L3eulaville WWTP; permit assigned to Vanessa Manuel)- Good plant and ORC. Some
I&I issues. No action recommended.
10) NCO036668 (Kenansville WWTP; permit assigned to Vanessa Manuel)- New plant (oxidation
ditch) operating well. No action recommended.
11) NCO039527 (New Hanover County/Walnut Hills VWVTP; permit assigned to Francis Candelaria)-
Discharge will eventually go to Wilmington Northside VWVTP. No action recommended.
12) NCO043796 (Columbus County/Acme Elementary; permit assigned to Francis Candelaria)- Good
ORC, no concerns. No action recommended.
13) NCO049743 (New Hanover County/Landfill WWTP; permit assigned to Bob Slew)- Some zinc
issue with landfill leachate. Constructed wetlands for polishing. No discharge after landfill closes. No
