A letter of request for an Authorization to Construct was received November 20, 1990 by
the Division and final plans and specifications for the subject project have been reviewed and found
to be satisfactory. Authorization is hereby granted for the construction of a pumping unit to pump
the effluent from the septic tank to the dosing tank, consisting of Duplex 20 GPM pumps, high
water alarms, approximately 20 - feet of 2 - inch PVC force main, a 1600 gallon dosing tank,
approximately 220 linear feet of 6 - inch gravity sanitary sewer, and appurtenances in accordance
with the approved plans and specifications with discharge of treated wastewater into an unnamed
tributary to Warrior Creek which is classified Class "C" waters in the Yadkin -Pee Dee River Basin.
This Authorization to Construct is issued in accordance with Part III paragraph B of
NPDES Permit No. NCO075515 issued March 20, 1990, and shall be subject to revocation unless
the wastewater treatment facilities are constructed in accordance with the conditions and limitations
specified in Permit No. NC0075515.
The sludge generated from these treatment facilities must be disposed of in accordance with
G.S. 143-215.1 and in a manner approved by the North Carolina Division of Environmental
Management.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may be
required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.

The Winston-Salem Regional Office, phone no. 9191761-2351, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility has
been installed in accordance with the NPDES Permit, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to the Permits and Engineering Unit,
P.O. Box 27687, Raleigh, NC 27611.
The Permittee shall employ a certified wastewater treatment plant operator to be in
responsible charge of the wastewater treatment facilities. The operator must hold a certificate of the
grade at least equivalent to the classification assigned to the wastewater treatment facilities by the
Certification Commission. Within thirty days after the wastewater treatment facilities are 50%
complete, the Permittee must submit a letter to the Certification Commission which designates the
operator in responsible charge.
A copy of the approved plans and specifications shall be maintained on file by the Permittee
for the life of the facility.
The sand media of the sand filter must comply with the Division's sand specifications. The
engineer's certification will be evidence that this certification has been met.
A leakage test shall be performed on the septic tank and dosing tank to insure that any
exfiltration occurs at a rate which does not exceed twenty (20) gallons per twenty-four (24) hour
per 1,000 gallons of tank capacity. The engineer's certification will serve as proof of compliance
with this condition.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Environmental Management in
accordance with North Carolina General Statute 143-215.6.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.

Dear Mr. Reavis:
A letter of request for an Authorization to Construct was received July 19, 1989 by the
Division and final plans and specifications for the subject project have been reviewed and found to
be satisfactory. Authorization is hereby granted for the utilization of the existing septic tank,
construction of two (2) 880 square feet sand filters with a 30-mil thick polyethylene liner, a 1032
gallon dosing tank with dual siphons, a 64 gallon chlorine contact tank, 384 linear feet of 6-inch
gravity effluent line with table chlorination and all pipes, valves and appurtenances to serve
Boomer Ferguson Elementary School with discharge of treated wastewater into an unnamed
tributary to Warrior Creek.
This Authorization to Construct is issued in accordance with Part III paragraph B of
NPDES Permit No. NC0075515 issued March 20, 1989, and shall be subject to revocation unless
the wastewater treatment facilities are constructed in accordance with the conditions and limitations
specified in Permit No. NC0075515.

The interior surfaces of the sand filter shall be completely lined with a 30-mil thick
polyethylene liner. Following installation and inspection of the sand filter liner, and prior to
placement of sand and gravel, certification of the liner's compliance with approved construction
specifications and the liner's integrity must be provided to the Division of Environmental
Management, Groundwater Section, by the project engineer.
Any groundwater quality monitoring, as deemed necessary by the Division, shall be
provided.
