The existing 36' L X 27` W reinforced concrete deck bridge on I-Beams with reinfarced concrete abutments is structurally deficient and will be replaced with a new 70' L X 33' W, single span, cored slab bridge at the same location. The exisring bridge has a sufficiency rating of 20.99. An off-site detour will be utilized during constructiov. There are no wetlands within the project area. This project proposes only 14 linear feet of permanent stream impacts at the outlet of a standard "V" ditch for embankment protection (i.e. rip rap will not be within the thalweg of the channel). The project drainage system consists of pipe outfalls that drain to existing roadside ditches. Additionally, the project requires the relocation of overhead utility lines on the south side of the bridge. Proposed Ripariav Buffer impacts include 3,289 square feet in Zone 1 and 1,362 square feet in Zone 2 to accommodate the bridge and the utility relocations. It should be noted that there is an historic property located to the north of the project, however, a"no adverse effecY' determination was recorded by the State Histaric Preservation Office (see attached letter). The Small Whorled Pogonia is listed as a Federally Threatened and Endangered Species within Guilford County aud its status, for this project, is currently "unresolved". A survey will be conducted in the spring of 2016 for habitat and species presence. The Bald Eagle is protected in every county in North Carolina under the Bald and Gold Eagle Protection Act; however, no suitable feeding/water som�ce is located within one mile of the project study area to support Bald Eagle habitat. Also, IVH•. Gary Jordan (USFWS) recently released the Programmatic Conference Opinion (PCO) for the recently listed Northern Long Eared Bat (NLEB), This opinion, states in part, that "... it is the Service's confe��ence opinion that NCDOT activities in eastern North Carolina (Divisions I-8), as proposed, are not likely to jeopardize the continued existence of the NLEB. Annual Avg Daily Traffic (veh/hr/day): Desi n/Future: 1100 Year: 2015 Existin : 1100 Year: 2015 General Project Narrative: Replacement of No. Bridge 400189 on SR 2719 (High Rock Road) over UT to Reedy Fork (Hardys Mill Pond) in Guilford County northwest of Gibsonville, NC. Proposed 70' long (Description of Minimization of Water by 33' wide single-span bridge to replace existing 36' long by 27' wide single-span bridge (reinforced concrete deck on I-beams). The proposed grade is about 2' above existing Quality Impacts) grade in the vicinity of the bridge and roughly matching existing by about 130' left of stream and about 80' right of stream (looking downstream). Bridge No. 400189 is within the Cape Fear River Drainage Basin and shall adhere to the Jordan Lake Riparian Buffer Rules. Proposed bridge to be located in same location as existing bridge. The proposed bridge will have no direct discharge into the water. No deck drains will be installed. Stormwater runoff is discharged at minimum practicable slopes, yielding minimum velocities and diffused where possible. A riprap dissipater pad is proposed at outlet of two proposed pipes. At two of the ditch outlets (12+gg Left & 13+2g Left), topographic constraints preclude diffusing discharge outside the buffer, so Class II Rip Rap has been proposed to protect the streambank as well as line the last 5' of the ditch to minimize the exit velocity, which the existing drainage does not benefit from. These two locations are substantially realignment of existing roadside drainage which is required to accommodate increased shoulder widths to satisfy minimum roadway design criteria. No additional travel lanes are proposed and the design calis for the minimum required roadway typical section based on traffic and safety considerations. All proposed stormwater runoff is discharged as far away from the stream and at the lowest velocities as practicable. The project will require the relocation of aerial utility lines and Duke Energy power poles which will also be used by TWC and AT&T. These lines will cross the stream perpindicularly on the south side of the bridge. Anticipate a 30 ft. wide permanently maintained corridor centered along the proposed OH line. There is also a historic property in the north quadrant of the project. Based on this review, there are no NR, SL, DE, or LL properties within the APE. "I'here is one SS (surveyed site), the Summers Log House and Barn (GP1843) located within the APE. Guilford County property and tax records werc reviewed which revealed one property �cross li•om Uie Summers Log House with a built-date of 1976. The APE sits in northeastern Guilford County, and consists of rural Y'orested and cleared farm land with residences interspersed along the roadways. It is not dear whether the SS property would be eligible for the National Register oP Historie Places (NRHP), therefore a swvey is ueeded to assess its condition and significance vid assess for any potential effects from project activities. A survey is required for Chis project. 1'he field survey was conducted in October of 2014. The Summers Log IIouse was still standiug but the barn and auother outbuilding were confirmed to be gone. NCDOT decided to evaluate the Summers Log House property for eligibility to the NRHP and contracted with Mattson, Alexander, aud Associates, Inc. to conduct the intensive suivey and evaluation. "I'he eligibiliry report was completed in Pebruaiy of 2015 with the consultant finding the Clapp Family Log House (Summers Log House) eligible for listing in the NRIIP under Criterion C as a representative example of mid-19°i century hewn, half-dovetailed log house and outbuilding construction in northeast Guilford Coimry. No: N/A Funding.• � State ❑ Federal Federal PeYmit Required? � Yes ❑ No Permit Type: NWP 3 or NWP 14 Project Description: The NCDOT proposes to replace Bridge No. 189 on SR 2719 (High Rock Road) over an Unnamed Tributary of Reedy Fork in Guilford County. Bridge No. 189 was built in 1963, and is considered to be structurally deficient and funcrionally obsolete. There may be minor ditch-line impacts, additional ROW is anticipated but not known at this time, and traffic will be maintained during construction on existing local roads (i.e. offsite detour). Temporary and permanent easements are anticipated as well; however, their exact locations and dimensions are not known at this time. To incorporate possible locations for additional ROW and easements, project length along SR 2719 (High Rock Road) will measure about 300 feet from either end of the bridge with a width of about 75 feet to either side of centerline. Based on these dimensions, the Area of Potential Effects (APE) measures approximately 93,539 square feet or about 2.15 acre, inclusive of the existing roadway and structure. SUMMARY OF CULTURAL RESOURCES REVIEW Bvief description of Yeview activities, results of review, and conclusions: A map review and site file search was conducted at the Office of State Archaeology (OSA) on Friday, September 19, 2014. A comprehensive archaeological survey at this particular bridge location has never been conducted, and no archaeological sites have been recorded within one-half (1/2) mile of the proposed project. Digital copies of HPO's maps (Ossipee Quadrangle) as well as the HPOWEB GIS Service (http://gis.ncdcr.gov/hpoweb/) were last reviewed on Tuesday, October 14, 2014. There is one (1) known historic architectural resource (GF1843 — Suminers Log House and Barn) located adjacent to the proposed project corridor; however, intact archaeological deposits associated with this resource are not anticipated within the footprint of the proposed project. In addition, topographic maps, historic maps (NCMaps website), USDA soil survey maps, and historic orthophotography were utilized and inspected to gauge environmental factors that may have contributed to historic ar prehistoric settlement within the project limits, and to assess the level of modern, slope, agricultural, hydrological, and other erosive-type disturbances within and surrounding the archaeological APE. Brief Explanation of why the available informaZion provides a reliable basis for reasonably predicting that there are no unidentified historic properties in the APE: This is a State-funded project that will require a Federal permit as well as permanent and temporary easements. Since NCDOT's existing ROW along SR 2719 (High Rock Road) measures 60 feet wide, there may be environmental impacts beyond what is currently owned/maintained by the NCDOT. From an environmental perspective, the APE consists of the rolling and undulating terrain characteristic of the North Carolina Piedinont, and is composed of several soil types: Cecil sandy loam, 2-6% slopes (CcB), Cecil sandy loam, 6-10% slopes (CcC), Chewacla sandy loam (Ch), and Cecil sandy loam, 10-15% slopes (CcD). Eroded soils are present just beyond the APE: Cecil sandy clay loam, 2-6% slopes, eroded (CeB2) and Madison clay loam, 10-15% slopes, eroded (McD2). Two (2) nearby secondary road improvement proj ects were reviewed by the Office of State Archaeology (OSA) for their potential to impact archaeological resources; however, both were considered "low probability" and surveys were not recommended (ER 02-10469 [SR 2776 (River Track Road)] and ER 03-2903 [SR 2775 (Michael Road]). Therefore, there have been no comparable properties reviewed and/or impacted within the proposed project's vicinity. Known archaeological sites in this corner of Guilford County tend to be located along major waterways like Reedy Fork and Buffalo Creek, both of which are located south of the project area. Given the degradation of soils within Guilfard County since the last county survey in 1977, a reconnaissance of the project area was conducted in order to gauge the integrity of the soils within the APE. Soil columns were taken at 30-meter intervals along the perimeter of the APE within areas of what was observed as moderate archaeological potential; unfortunately, soil deflation is quite evident. Additional columns were taken in the adjacent agricultural fields (corn-NE, tobacco-SE, and fallow-SW); typical overlying A-horizons and/or plowzones (Ap) were not detected within the profiles, which consisted entirely of clay loam and clay strata. Various fragments of quartz were observed on the surface within the tobacco field with none being cultural in nature. Exposed rock was rather evident in the Northwest Quadrant, with overgrown rock piles located in the wooded section of this quadrant; these piles are interpreted as evidence of field-clearing activities and are not considered archaeological in nature. Based on the overall topography, increased soil deflation, hydric soils, utility disturbances, and driveways/field access roads within the APE, it is believed that the project area, as depicted, is unlikely to contain intact and significant archaeological resources. Therefore, there should be no archaeological survey required for this project. If design plans change or are made available prior to construction, then additional consultation regarding archaeology will be required. At this time, no further archaeological work is recommended. If archaeological materials are uncovered during project activities, then such resources will be dealt with according to the procedures set forth for "unanticipated discoveries," to include notification of NCDOT's Archaeology Group.
