Phase II Expansion of Wastewater
Treatment Plant
Dear Mr. Vestal: Forsyth County
A letter of request for Authorization to Construct was received October 23, 1986,
by the Division and final plans and specifications for the subject project have been
reviewed and found to be satisfactory. Authorization is hereby granted for the
construction a 16,500 GPD wastewater treatment facility addition to the Forest Ridge
Subdivision Wastewater Plant to provide 33,000 GPD total capacity, said addition
consisting of a 20,625 gallon capacity aeration tank (sized for 30 hours detention
time) installed in parallel with the Phase I plant, dual 140 CFM blowers to serve
the new plant, a 3,000 gallon capacity sludge holding tank, a-4,450 gallon capacity
clarifier, a 515 gallon baffled chlorine contact tank with tablet chlorinator, a 505
gallon post -aeration tank with air diffuser, recording effluent flow meter with
totalizer, and all associated piping, valves, fittings, and appurtenances to serve
the Forest Ridge Subdivision in Forsyth County.
This Authorization to Construct is issued in accordance with Part III paragraph
C of the NPDES Permit No. N000063720 issued October 8, 1986, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0063720.
The Permittee must employ a certified wastewater operator in accordance with
Part III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143.215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.

-,' The Winston-Salem Regional Office, telephone number 919/761-2351 shall be
j notified at least twenty-four (24) hours in advance of start-up the installed
facilities so that an in -place inspection can be made of said facilities prior to
operation. Such notification to the Regional Supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding
State Holidays.
In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective action
as may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
Pumping capacity of the influent pump station shall be upgraded to handle the
additional design flow when average daily flow to the plant (monthly average) reaches
50 percent of total design capacity (i.e. 16,500 GPD)
