Dear Mr. Vestal
A letter of request for Authorization to Construct was received June 9, 1986, by
the Division and final plans and specifications for the subject project have been
reviewed and found to be satisfactory. Authorization is hereby granted for the
construction of a 0.0165 MGD wastewater treatment facility consisting of a 30 GPM
influent pump station with dual grinder pumps and high-water alarm, a 20,625 gallon
aeration tank sized for 30 hours detention time, dual 140 CFM blowers, a 4,450 gallon
clarifier, a 3,000 gallon sludge holding tank, a 515 gallon baffled chlorine contact
tank with tablet -type chlorinator, a 515 gallon post aeration tank with air diffuser,
an effluent flow recorder, and all associated piping, valves, fittings and
appurtenances to serve the Forest Ridge Subdivision in Forsyth County.
This Authorization to Construct is issued in accordance with Part III paragraph C
of the NPDES Permit No. N0063720 issued October 8, 1986, and shall be subject to
revocation uAless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0063720.

III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143.215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.

notified at least twenty-four (24) hours in advance of operation of the installed
facilities so that an in -place inspection can be made of said system prior to
start-up. Such notification to the Regional Supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding
State Holidays.
In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the Peicmittee-shall take such immediate corrective action as
may be required by this Division, including the construction of additional wastewater
treatment and disposal facilities.
