Director Thomas Golden 5905 Paragon Cir Durham, NC 27712 NORTH CAROLINA Environmental Quality April 05, 2024 SUBJECT: COMPLIANCE EVALUATION INSPECTION NPDES Wastewater General Permit: NCG550000 Certificate of Coverage: NCG550035 Single Family Residence Wastewater Treatment System 5905 Paragon Cir, Durham, NC 27712 Durham County Dear Mr. Golden: On March 15, 2024, Division of Water Resources (DWR) Raleigh Regional Office (RRO) staff Jesse Barnes attempted to conduct a routine compliance evaluation inspection at the single family residence wastewater treatment system located at 5905 Paragon Cir, Durham, NC 27712 in reference to NPDES General Permit NCG550000 and Certificate of Coverage NCG550035. The residence appeared to be vacant during the site visit and DWR staff were unable to locate any wastewater treatment components, so an inspection was not completed. Any determined permit violation(s) and item(s) of concern are listed below, along with other observations. Inspection findings are detailed in the attached Water Compliance Inspection Report. Item(s) of Concern 1. Part II, Sections B. (8.) and D. (8.) of the subject permit require the permittee to provide, within a reasonable time, any information the Permit Issuing Authority may request to determine compliance with the subject permit and allow inspection of the facility at reasonable times. Using contact information provided by the permittee, DWR staff requested access to the subject facility to conduct a compliance evaluation inspection and a response from the permittee via USPS certified mail (tracking number: 7017 2680 0000 2219 6630) on March 7, 2023. DWR staff also attempted to reach the permittee by phone and in person at the residence (left contact letter at the front door) using contact information provided by the permittee. The permittee has not responded to DWR staff requests. 2. According to DWR GIS data and in -person observations, the subject facility effluent appears to discharge to a UT to Cabin Branch (Stream Index: 27-1-21-7), classified as WS-IV and NSW of the State within the Neuse River Basin (Sub -basin: 03-04-01), which differs from the receiving waters listed on previous Certificates of Coverage and in DWR's internal files. Other Observations The subject facility is located at 5905 Paragon Cir, Durham, NC 27712. Version 7.0 of NPDES General Permit NCG550000 and Certificate of Coverage NCG550035 became effective November 20, 2020, and will expire at midnight on October 31, 2025. The permitted facility is a single family wastewater treatment system with unknown components.
