3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application:
SEE ATTACHED SHEETS R1 THRU R5. The existing Concrete Dam is located within the Highlands Falls Country Club
residential neighborhood. The Concrete Dam is located on the Cullasaja River, at the confluence of Ammons Branch and
Saltrock Branch. The existing Concrete Dam is in need of mantenance and repair as described herein.

THE PURPOSE OF THE PROJECT IS TO PERFORM REPAIRS TO THE EXISTING CONCRETE DAM TO ENSURE
SAFETY AND STRUCTURE INTEGURITY.
3e. Describe the overall project in detail, including the type of equipment to be used:
SEE ATTACHED SHEETS R1 THRU R5.
THE HIGHLANDS FALLS COUNTRY CLUB DAM (MACON-007) IS SUBJECT TO THE REGULATIONS OF THE NORTH
CAROLINA DAM SAFETY ACT OF 1967. DOWNSTREAM HAZARDS INCLUDE PUBLIC AND PRIVATE ROADWAYS,
PRIVATE PROPERTY, ACCESS DRIVEWAYS, AND RESIDENCES. THE PROPOSED REMEDIATION PROVIDES FOR
LONG TERM STABILITY OF THE CONCRETE STRUCTURE. THE PROPOSED WORK SATISFIES THE REQUIREMENTS
OF THE REGULATIONS BY MAINTAINING THE SPILLWAY CAPACITY AND STABILIZING THE SPALLED CONCRETE.
IMPROVEMENT MEASURES INCLUDE:
1. REMOVAL OF DELETERIOUS CONCRETE.
2. PREPARATION OF CONCRETE SUBSTRATE.
3. APPLICATION OF BONDING AGENTS.
4. APPLICATION OF STRUCTURAL GROUT TO RESTORE DAM FORM.
5. REMOVAL OF EXISTING BOTTOM DRAIN GATE STRUCTURE.
6. INSTALLATION OF NEW BOTTOM DRAIN GATE.
7. INSPECTION AND REFURBISHMENT OF UPPER LEVEL GATE.
WORK WILL BE PERFORMED WITHIN A REGULATED FLOODPLAIN. DAM MODIFICATIONS MUST RESTORE
ORIGINAL DAM CREST GEOMETRY AND NOT IMPACT FLOODPLAIN DELINEATION. THEREFORE, ELEVATIONS AND
DIMENSIONS MUST BE CONSISTENT WITH THE CURRENT FLOODPLAIN DELINEATION MODEL.
SANDBLAST AGGREGATE CAPTURE WILL BE REQUIED. THE CONTRACTOR SHALL TAKE ALL NECESSARY
PRECAUTIONS TO INSURE THAT SANDBLAST AGGREGATE IS CAPTURED AND NOT ALLOWED TO ENTER
SURFACE WATERS OR OTHERWISE CONTAMINATE THE TROUT BUFFER.
FRESH CONCRETE SHALL NOT BE ALLOWED TO CONTACT SURFACE WATERS.
All temporary Erosion Control Measures shall be in accordance with the latest edition of the NC EROSION and SEDIMENT
CONTROL PLANNING and DESIGN MANUAL, and in accordance with the Plans by Garner Engineering.
No disturbance within existing Vegetated Trout Buffer. Provide Turbidity Curtains around work area to maintian water quality.
