As the current owner you were recently named as one of the "Permittee(s)" on the NC
Department of Environmental Quality- Division of Water Resources 401 Water Quality
Certification (3890) for the proposed Middleton Subdivision (DWR# 15-0917, enclosed).
This was an administrative error on our part, and we sincerely apologize as MI Homes is to
be responsible for compliance with all conditions set forth in the permit and ultimately the
"Permittee".
In order to rectify and remove your name(s) as the "applicant" and "Permittee", can you
please acknowledge, by checking the box below, that Ml Homes of Raleigh, LLC, may
apply on your behalf for the NC Department of Environmental Quality- Division of Water
Resources 401 Water Quality Certification, and return this letter to Mr. Jeremy Medlin.

for the NC Department of Environmental Quality- Division of Water Resources
401 Water Quality Certification (3890), which will then cause the 401 Water
Quality Certification (3890) to be re -issued to MI Homes of Raleigh, LLC.
