Dear Mr. York:
On February 29th, 2024, Ron Boone, of the Winston-Salem Regional Office of the NCDWR, came
to your home to conduct a compliance evaluation inspection of your single-family wastewater
treatment system, but you were not available at the time for an inspection. You gave Mr. Boone
a phone number to call you and schedule an inspection in the future. He's since called you several
times and not been able to reach you.
Please contact Mr. Boone by phone at 336-776-9690 or by email at ron.boone@deg.nc.gov to
schedule a date and time that you can meet him on site to perform the inspection, which we are
required to do a minimum of every five years. Thank you for your attention to this matter.
