Box 1180 Rocky Mount, NC 27802 RE: Project Name: City of Rocky Mount Fire Station #2 Acres Approved: 1.21 Application ID: PA-002915 Permit Number: NASH-2024-00103 Address: 824 Grace Street City: Rocky Mount County: Nash River Basin:Tar-Pamlico Stream Classification: NSW: Nutrient Sensitive Waters;WS-IV:Water Supply IV Plan Type: New/Institutional Dear Sir or Madam, This office has reviewed the subject erosion and sedimentation control plan. We find the plan to be acceptable with modifications and hereby issue this Letter of Approval with Modifications. The Modifications Required for Approval are listed on the attached page.The Certificate of Approval must be posted at the job site. This plan approval shall expire three (3)years following the date of approval, if no land-disturbing activity has been undertaken, as is required by Title 15A NCAC 4B .0129. As of April 1, 2019, all new construction activities are required to complete and submit an electronic Notice of Intent(eNOI)form requesting a Certificate of Coverage (COC) under the NCG010000 Construction General Permit. After the form is reviewed and found to be complete,you will receive a link with payment instructions for the$120 annual permit fee. After the fee is processed,you will receive the COC via email. As the Financially Responsible Party shown on the FRO form submitted for this project,you MUST obtain the COC prior to commencement of any land disturbing activity. The eNOl form may be accessed at deq.nc.gov/NCG01. Please direct questions about the eNOl form to the Stormwater Program staff in the Raleigh central office. If the owner/operator of this project changes in the future,the new responsible party must obtain a new COC. Title 15A NCAC 4B .0118(a) and the NCG01 permit require that the following documentation be kept on file at the job site: 1. The approved E&SC plan as well as any approved deviation. 2. The NCG01 permit and the COC, once it is received. 3. Records of inspections made during the previous 12 months. Also,this letter gives the notice required by G.S. 113A-61.1(a) of our right of periodic inspection to ensure compliance with the approved plan. North Carolina's Sedimentation Pollution Control Act is performance-oriented, requiring protection of existing natural resources and adjoining properties. If,following the commencement of this project, the erosion and sedimentation control plan is inadequate to meet the requirements of the Sedimentation Pollution Control Act of 1973 (North Carolina General Statute 113A-51 through 66),this office may require revisions to the plan and implementation of the revisions to insure compliance with the Act. Acceptance and approval of this plan is conditioned upon your compliance with Federal and State water quality laws, regulations, and rules. In addition, local city or county ordinances or rules may also apply to this land-disturbing activity. This approval does not supersede any other permit or approval. Please note that this approval is based in part on the accuracy of the information provided in the Financial Responsibility/Ownership Form, which you provided. You are requested to file an amended form if there is any change in the information included on the form. This permit allows for a land- disturbance, as called for on the application and plan, not to exceed the approved acres. Exceeding the acreage can be a violation of this permit and may require a revised plan and additional application fee. In addition, it would be helpful if you notify this office of the proposed starting date for this project. Modifications Required for Approval: 1. Please indicate total disturbed acreage on appropriate plan sheet(s).The noted disturbed area must exactly match that which is reported on the Financial Responsibility/Ownership (FR/O) Form. If proposed disturbed area differs from what was reported on the submitted FR/O form, please revise FR/O form and submit with new notarized "wet" signature. (GS 113A-57(3), 15A NCAC 04B .0105, 15A NCAC 04B .0106(5)) a.The FRO form, narrative and plan set mention different values for the total disturbed acreage(1.21 acres, 1.13 acres and 1.135 acres respectively). 2. Add the following statement to the construction sequence: "Erosion and Sediment Control (E&SC) permit and a Certificate of Coverage (COC) must be obtained before any land disturbing activities (including timbering and demolition) occur." The COC can be obtained by filling out the electronic Notice of Intent(e-NOI)form at deq.nc.gov/NCG01. Please note, the e-NOI form may only be filled out once the plans have been approved. A copy of the E&SC permit,the COC, and a hard copy of the plan must be kept on site, preferably in a permits box, and accessible during inspection. 3. Add a requirement to the construction sequence to contact the DEMLR Raleigh Regional Office at least 48 hours prior to commencing the land-disturbing activity.The contact number is (919) 791-4200. (15A NCAC 04B .106(b)) 4. Add the following statement to the construction sequence, "When the project is complete,the permittee shall contact DEMLR to close out the E&SC Plan."After DEMLR informs the permittee of the project close out, via inspection report,the permittee shall visit deq.nc.gov/NCG01 to submit an electronic Notice of Termination (e-NOT). A$100 annual general permit fee will be charged until the e- NOT has been filled out. 5. Add the following statement to the construction sequence: "Self-inspections for erosion and sedimentation control measures are to be performed at least once every seven calendar days and within 24 hours of every rain event of greater than 1 inch. Any needed repairs shall be made immediately to maintain measures as designed. All ESC measures shall be maintained as specified in the construction details on this plan. A rain gauge shall be installed at the project site for monitoring." (GS 113A-54.1 (e), 15A NCAC 04B.0131, NCG01 Section III.A., NCG01 Section III.B.) 6. Self-inspection forms were updated on 07/01/2020. Please ensure new version of forms is used. (GS 113A-54.1 (e), 15A NCAC 046.0131, NCG01 Section III.A., NCG01 Section III.B.) a.Add the following statement to the note about self-inspections: "The contractor shall conduct self- inspections of the erosion and sedimentation control measures and compete the following combined self-inspection form found on the DEMLR website: https://deq.nc.gov/about/divisions/energy-mineral- land-resources/erosion-sediment-control/forms.Twelve months of complete inspection forms shall be kept on-site and available for inspection at all times.
