The project was reviewed with DOT staff Phill Suggs and Bojan Cvjetic and Mitch Leonard (contractor). Erosion control
measures were repaired, but additional temporary cover was needed as recommended by Roadside Environmental.
NPDES records were completed satisfactorily.

Comment: A track hoe operator inadvertantly tracked through a berm which released water, silt and turbidty into
Salem Creek where bridge pile drilling was underway. The release was reported as required. This was a
follow-up inspection to ensure EC measures were repaired. The berm was repaired but further cover needs
to be estabilished on adjacent cut slopes leadinq down to Salem Creek.
