Permit Status Yes No NA NE
# Is the current resident in the home the Permittee? 0 ❑ ❑ ❑
# If not, does the resident rent from the Permittee? ❑ ❑ 0 ❑
Change of Ownership form needed? (Mail the form with the inspection letter) 0 ❑ ❑ ❑
# Is there an inspection and maintenance agreement with a contractor? ❑ ❑ 0 ❑
If YES, who is the contractor (include contact info)?
Historically this system was maintained by Water Management (Sid Gaskins 828-244-9765 or
Sid@watermanementnc.net) and TCW Wastewater (Brandon Long 704-351-4049 or
Brandon@tcwwastewater.com) was used for recent repairs.
Comment: The home is under contract for sale. MRO staff will need to follow-up with the next owners to
ensure they received a copy of the Change of Ownership Form sent to present owner with this
report.
Septic Tank Yes No NA NE
*** The septic tank and filters should be checked annually and pumped/cleaned as needed.'
Is all wastewater from the home connected to the septic tank? ❑ ❑ ❑
# Does the permittee/resident know where the septic tank is located? ❑ ❑ ❑
Has the septic tank been pumped in the last 5 years? ❑ 0 ❑ ❑
If YES, describe if known and proof (include date pumped):
For past several years occupancy by various owners has resulted in only two adults utilizing the system.
During this inspection sludge judge showed 13" of sludge volume in 1 st compartment of the septic tank vs. 4'
total volume and did not indicate the need for pumping. Some floating scum was also noted. Percentage of
sludge to total volume ratio was less throughout the rest of the system.
# Does the septic tank have an EFFLUENT FILTER or SANITARY T? ❑ ❑ ❑
If FILTER, when was the filter cleaned and by who? 12/13/2022 by
contractor.
Comment: Both filters, at septic tank and recirculating pump tank, were pulled and rinsed off into the
septic tanks' 1 st compartment.
Sand Filter/Treatment Pods Yes No NA NE
*** Accessible sand filter surfaces shall be raked/leveled every 6 months and vegetative growth shall be
removed manually. ***
# Is system something other than a sand filter? 0 ❑ ❑ ❑
# If YES, what kind? (examples - Peat, Textile or brand name - Advantex, etc.) Textile
# Does the permittee know where the sandfilter is located? 0 ❑ ❑ ❑
Does the sandfilter require maintenance? ❑ ❑ On
If maintenace is required, explain:
This system historically appears to have more bio-growth than MRO staff has observed in other Single Family
Residence systems. Watch for bridging (build-up) between textile gaps - a couple were removed by the
technician during this site visit and may have been related to insect activity. Distribution caps appear to need
rinsing approximately every 6 months, with routine maintenance.
Comment: As stated above.

Disinfection UV Yes No NA NE
*** UV unit shall be checked weekly. Lamps/sleeves should be cleaned/replaced as needed to ensure
proper disinfection. ***
Is UV working? ❑ ❑ ❑
Has the UV Unit been serviced and bulbs cleaned? 0 ❑ ❑ ❑
# Who completes the weekly check for the UV? ( Non -Discharge) Owner - Discuss
with technician or
manufacture for
safe weekly check
of UV.
Comment: The entire UV disinfection unit was replaced during this inspection. TCW technician noted at
the second October site visit that the UV system was not operable, the replacement unit was
received on 12/9/2022 and installed on 12/13/2022.
MRO staff recommends the owner(s) discuss with their technician or system manufacturer for
safe weeklv check of UV. For eve safetv - do not view bulb.
Pump Tank Yes No NA NE
*** All pump and alarm sytems shall be inspected monthly. (Non -Discharge) ***
Is the pump working? 0 ❑ ❑ ❑
Is the audible and visual high water alarm operational? 0 ❑ ❑ ❑
# Does the permittee know how to check the pump & high water alarm? ❑ 0 ❑ ❑
# Last functional test: October 26, 2022
Comment: Per phone communications with the owner(s) after the May 2022 inspection, a new pump also
failed and reduced power was determined to be the reason for pump and high-water alarm
issues. MRO staff received follow-up email on August 1 st, 2022, stating an electrician
completed power repairs on Friday, July 29th, 2022, but drip -line repairs were still needed.
Visual and audible alarms were viewed at the Oct 26th. 2022. site visit and durina the
December 13th, 2022, site visit TCW staff tested all electrical communication between the
pump tank floats and the control box. Technicians from both contract companies (TCW and
Water Management) stated there is a programmed delay for the high-water float to trigger the
alarms. Most systems today also have installed telemetry or an auto -dialer system to call/text
the owner and/or contractor when a high-water float is triggered. Neither is required but
recommended and is at the discretion of the new owner.
Drip or Irrigation
*** Irrigation sysetm shall be inspected monthly to ensure system is free of leaks and equipment is operating
as designed. ***
# Type of system (DRIP or IRRIGATION):

Permit: WQ0032548 Owner - Facility: Jeffrey E Brink
Inspection Date: 12/13/2022 Inspection Type: Compliance Evaluation Reason for Visit: Follow-up
Comment: This facility has a reduced setback waiver to a property line for the drip field. TCW staff
repaired several line breaks in all zones of the drip field during the October 26 & 27, 2022, site
visits. After installation of the UV unit, the remaining dripline repairs were made on December
13th, 2022. At the time of inspection, MRO staff did not observe additional drip line breaks.
Routine field maintenance, including mowing, reseeding and appropriate soil replacement for
holes / depressions are the responsibility of the permittee and typically are not services
provided by wastewater system contractors.
General Yes No NA NE

# Has resident had any sewage problems? ❑ ❑ ❑
If YES, explain:

Does the system match the permit description? ❑ ❑ ❑
