TIP No. R-2303 = Sampson County DeQr Ms. Chapman: We are in receipt of the subject Notice of Deficiency dated October 12, 2015. We have reviewed the issues and have responded as follows. The Contractor, Fred Smith Co. and its subcontractor Wellons began laying a pipe at Site 7 the afternoon of the 7th. The DOT staff had inspected the operation earlier and subsequent to that time, Wellons began receiving additional wuter into their work area. An earthen dam w4s placed in the stream to prevent wctter that was being pumped around from back flowing into the site. When this was found by DOT inspection staff, Wellons was directed to remove the dam and refused. Fred Smith Co. insisted on.waiting until Mason arrived for his input. In addition, Wellons�wus preparing to IQy pipe at Site 6 on the 8th. Upon arrival with the Agencies, excessive turbidity w4s discovered leaving the pro ject along 5501 Barbados Boulevard, Castle Hayne, NC 28429 Phone (910) 341-2000 Fax (910) 675-0143 with silt loss. The operations were immediately shut down until the area was restored and proper erosions measures were in place. The DOT Resident Engineer and the inspection staff held a staff ineeting on Monday October 12, 2015 to review the events and the details shown in the NC�OT BMP flip book. They specifically called out the details for impervious dikes and the prohibition uguinst using Qny erodible material even if wrapped in geotextile. The project staff is providing additional oversite to the pipe instullation crew to prevent recurrences. �ivision Engineer Karen Collette, PE has scheduled a meeting with the pro ject management team of Fred Smith Co. and the subcontractor Wellons. During thQt meeting the �epartment will stress the importunce of ineeting all of the project commitments and we will advise the Contractor that operations throughout the project will receive additional oversite. Ms. Janet Russell will be assigned these two projects and will �perform �a review on some portion euch week. Her findings will be shured�`with the pro ject staff and Division Construction Engineer Chad Kimes. The Department regrets this deficiency and will continue to work to eliminate any future recurrence. Sincerely, Karen E.
