Hi Ryan. Our sediment removal and site restoration has been completed for the loss on AB. Thomas and I were on site
Friday last week to review, and the sites required some additional work. The additional work has been completed
now. Please advise if you have any questions.

The STIP number is U-2579AB. The DWR number is 19980260 (WQC004131). The DWR number includes many phases
of the Winston Salem Northern Beltway, and there have been many versions.

Amy, these are the pictures of our loss. The pond doesn't have a permit number because it is really off of our project.

Location: Site 1 (Sta-Y4RPB-Rt 20+00)
Rain event: 2.94"
Estimated Loss: 10 CYS
Did the Loss Occur in the Stream/Wetland: Yes
Length Of Loss: 100'
Cause: SDO overwhelmed
Corrective Action: Repair Type B basin and clean out rock lined ditch.
Timeline for Repair: 1 day

Rain event: 2.94"
Estimated Loss: 30 CYS
Did the Loss occur in the Stream/Weland: yes.
Length Of Loss: 500'
Cause: Inlet protection overwhelmed
Corrective Action: Repair inlet protection, KRG is remove sediment by vacuuming the silt out of the pipe, and cleaning
the silt from the JS, place inlet ring around pipe.

Rain Event: 2.94"
Estimated Loss: 10 CYS. The sediment is in the non -jurisdictional ditch and the pond/wetland.
Did the Loss occur in the stream/wetland: Yes.
Length of Loss: 200'
Cause: Silt Fence blew out.
Corrective Action: Grade area to drain water away, and replace silt fence, and SDO'S. Vacuum silt deposition from pond
to the maximum extent practicable while minimizing disturbance.
