GENERAL INFORMATION
General Program Information:
• HWA, ERP, SUO, and IWS are currently up-to-date
• Per the HWA approval letter in March 2022, I submitted a revised AT
(3/29/22), but has not been approved
• LTMP was submitted (3/31/22), but has not been approved
• Corrections to the program info database are noted on the 12/22/23 copy:
PCI and Audit dates, LTMP submittal date, Regional contact
General Permit Information:
• No name changes in 2023
• Charleston Forge IUP #0004 -expiration December 31, 2026

■ Au incomplete flow report was submitted and required re-
submission by November 30. The completed report was not
submitted until after 45 days, in 2023.

Batch discharger. Flow meter is read every production day and
recorded. Zero flow recorded on production days is included in
calculated averages.
Erroneous flow metering led SIU staff to notify Town of problem
with flow meter. They were able to determine it was false by
looking in the tanks and noting the water levels had not changed.
Service technician could not determine cause. Source of flow
meter malfunction remained unknown during 2023 and
replacement was being sought. This erroneous data was excluded
from the IDSF calculations.
