Dear Mr. Seawright:
On October 25, 2005, Jenifer Carter of this office perfoinied a General Permit Inspection on the
wastewater treatment system at the abovementioned address in Davie County,North Carolina. Mr.
Seawright was present for the inspection. This type of inspection consists of two basic parts: a review of
self-monitoring data, and an on-site inspection of the facility and its discharge.
You hold a General Permit to discharge domestic wastewater from a single family residence under
the National Pollutant Discharge Elimination System (NPDES). You are found to be in compliance with
your General Peiiuit No. NCG550244. There was no evidence of wastewater discharge in the area where
the effluent pipe is located(an open field), although the effluent pipe could not be located. The nearest
surface waters are at least a half mile away. Please refer to your permit for monitoring requirements. If
you notice any future discharge, please monitor accordingly the effluent characteristics listed in the
permit. All system components appeared to be functioning properly. If you have any questions
