We have not obtained an official name change form from her. Her husband had a lot of health problems and died
recently. She is elderly, and said she doesn't do email. If we need an official form completed,we will have to mail it to
her. I wasn't sure if we needed a form or not since her husband has passed and there wont be any signature from the
previous permittee.
She said her husband had been taking care of everything with the wastewater system. I wonder if that is why the fee is
overdue. Not sure why she didn't sign for the letters to be received.The correct mailing address is:

Also, is the correct address 4488 NC Highway 801 North, Mocksville NC 27028? That is what is in BIMS,and also on two
different certified letters for late payment of annual fees that we sent back in 2016...that were returned to us. I don't
want to process the name change paperwork until I have the correct address.

I conducted an inspection last week at the subject SFR facility.The name on the last certificate of coverage was Mr.
Perry Seawright. He recently passed away. His wife still resides there. Please change the name from Perry Seawright to
Barbara Seawright.
