Dear Mr. Bradshaw:
The Division of Water Quality's NPDES Unit has reviewed your request for an Authorization to
Construct permit for the Heritage Meadows WWTP. The Division finds the proposed improvements
to be satisfactory and hereby grants authorization for the following:
Installation of 1 module/2 lamp ultraviolet disinfection system,
Retention of tablet chlorination as a means of back-up disinfection, and
Any other piping, valves, control equipment, and appurtenances.
This Authorization to Construct is issued in accordance with NPDES Permit No. NC0047279,
issued January 31, 2000, and shall be subject to revocation unless the wastewater treatment
facilities are constructed in accordance with the terms and conditions of the permit.
The Permittee shall notify the Water Quality staff of the Raleigh Regional Office, telephone number
(919) 471-4700, at least forty-eight (48) hours prior to operation of the installed facilities, so that
an in -place inspection can be made. Notification shall be made during the normal office hours of
8:00 a.m. until 5:00 p.m., Monday through Friday.
Engineer's Certification
Upon completion of construction and prior to operation of the modified facility, the Permittee shall
provide the Division with a certification (copy enclosed) from a professional engineer registered in
North Carolina certifying that the permitted facility has been installed in accordance with the
NPDES Permit, this Authorization to Construct, and the approved plans and specifications. The
Certification should be submitted to: NCDENR / DWQ, NPDES Unit, 1617 Mail Service Center,
Raleigh, North Carolina 27699-1617.
Requirements for Certified Operator
Upon classification of the facility by the Certification Commission, the Permittee shall employ a
certified water pollution control treatment system operator to be in responsible charge (ORC) of the
water pollution control treatment system. The operator must hold a certificate of the type and

grade at least equivalent to or greater than the classification assigned to the water pollution control
treatment system by the Certification Commission. The Permittee must also employ a certified
back-up operator of the appropriate type and grade to comply with the conditions of Title 15A,
Chapter 8G, .0202. The ORC of the facility must visit each Class I facility at least weekly and
each Class II, III, and IV facility at least daily, excluding weekends and holidays, and must
properly manage and document daily operation and maintenance of the facility and must
comply with all other conditions outlined in Title 15A, Chapter 8G, .0204. Once the facility is
classified, the Permittee must submit a letter to the Certification Commission which designates the
operator in responsible charge within: (A) Sixty calendar days prior to wastewater being introduced
into a new system or (B) within 120 calendar days of the following, (i) after receiving notification of
a change in the classification of the system requiring the designation of a new ORC and back-up
ORC or (if) a vacancy in the position of ORC or back-up ORC.
Additional Requirements
The Operational Agreement between the Permittee and the Environmental Management
Commission is incorporated herein by reference and is a condition of this Permit. Noncompliance
with the terms of the Operational Agreement shall subject the Permittee to all sanctions provided
by G. S. 143-215.6 for violation of or failure to act in accordance with the terms and conditions of
this Permit.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Water Quality in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from complying
with any and all statutes, rules, regulations, or ordinances which may be imposed by other
government agencies (local, state, and federal) which have jurisdiction.
The Permittee shall maintain a copy of the approved plans and specifications on file for the life of
the facility. One (1) copy of the approved plans and specifications is enclosed for your records.

In accordance with North Carolina General Statute § 90A-37, the Water
Pollution Control System Operators Certification Commission is required to
classify all water pollution control systems. The classification of Biological
Water Pollution Control Treatment Systems, found at 15A NCAC 8G .0302,
was adopted by the Commission to classify water pollution control systems.
The classification of a facility is determined by permitted flow and biological
nutrient reduction limits.
The Water Pollution Control System Operators Certification Commission has
determined that the subject facility is classified as a Grade 2 Biological
Water Pollution Control Treatment System.
As required by 15A NCAC 8G .0202 and your permit, you must designate a
Grade 2 or higher Operator in Responsible Charge (ORC) and a Back-up
Operator with at least a Grade 1 certification of the appropriate type and grade
for this system. Please complete and return the enclosed designation form, so
we may update our database of any changes at your facility by
November 27, 2000. It has been detemrined that the Authorization to
Construct that you recently received requires no classification change at this
facility.

PART IV - EVALUATION AND RECOMMENDATIONS
The Raleigh Regional Office has reviewed the current submittal package requesting
Authorization to Construct (ATC) a UV disinfection system onto the Heritage Meadows
WWTP facility. Our staff has reviewed our files of the above referenced site, as well as the
plans and specifications submitted, in order to make the following comments and
recommendations for the request.
1) The RRO recommends that consideration be given to maintaining a chlorine tablet
disinfection system as a backup rather than require that a spare UV unit be kept on site. A
spare UV unit can be easily damaged (exposure to elements, mishandling, etc.) during the
several years that it may be stored on site as a backup, and therefore may not even be
operational if needed. However, a tablet chlorinator backup is virtually damage -proof, and
will operate properly whenever needed.
2) The plan sheet is very vague as to what exactly is being provided. The specifications
state that the level will be controlled by an effluent weir, however, the plans only show a weir
on the inlet end of the UV module channel. There is a box with a circle shown on the effluent
end of the UV module channel. This box and circle are not described, and their function is
unclear. In addition, there are two large "vaults?" that are provided on each end of the UV
module. The function of these "vaults", as well as the angled line on the top of them (hinged
opening lid?) is not clearly described. The engineer should be required to provide more
accurate plans for review.

With the above comments taken into consideration, the RRO recommends that the subject
project be awarded an ATC as requested, pending NPDES Permitting Unit satisfaction of all
requirements.
