Close -Out Status

On June 11, 2015, the Division of Water Resources (DWR) received a close -out packet from you on
behalf of the Division of Mitigation Services (DMS) requesting approval to close -out the subject site
from monitoring. In April 2014, Eric Kulz with DWR visited the subject site to assess the vegetation
and overall site conditions. On August 4, 2015, DWR received a revised close -out packet for the subject
site. On September 23, 2015, Katie Merritt with DWR requested additional information to be submitted
with the close -out request. On September 30, 2015, Ms. Merritt received all items needed to assess the
site for close -out.
As a result of Ms. Merritt's review of the close -out request, the following mitigation is approved:
