I am aware that there are significant penalties for submitting false information. 2�t L'i'J- 11atIL-4-el', 12�''— ­ - !K Pretreatment Coordinator Date TOWN OF YADKINVILLE NPDES # NCO020338 2023 PAR NARRATIVE GENERAL INFORMATION: PROGRAM INFORMATION: • AT, LTMP, SUO, ERP, IWS, HWA and Permits are up to date. • All 4 SIU permits were modified as needed on 1/1/23 to align detection levels to State PQL's. Permits were approved on 1/17/23. • Unifi permit #0002 and Unifi permit #0005 were submitted and approved for renewal on 4/1/24. • Austin Electrical Enclosures permit # 0006 was submitted and approved for renewal with 2 changes. A name change and zinc limit change from concentration to a mass- based limit. Permit effective date 4/1/24. • New NPDES Permit was received 1/28/2020. Effective date 3/1/2020. A WER for copper was submitted NCDEQ on 4/21/2021 . A Final NPDES Permit Modification was received from NCDEQ on 6/3/2022 with a higher limit for copper as a result of performing the WER study. • A 5-year compliance schedule for copper was included within the new Permit. • Currently working on WWTP permit (NC0020338) renewal. Permit expires 2/28/25. • An updated LTMP was submitted on 6/15/20 and approved on 9/14/2020. • An updated IWS was submitted on 11/22/2021 and approved on 6/16/2022. • An updated HWA was submitted on 8/16/2022. • A 5-year audit was performed by Tricia Lowery from the WSRO on 4/27/23. • Civil Penalties collected for 2023 were $500.00. • The allocation table provided with the PAR is from the 1/24/2024 HWA submission. • The PPID date for the HWA received date and the last Audit date were changed to reflect the most current dates. All other dates listed were correct. PERMIT INFORMATION THE AUSTIN COMPANY IUP # 0006 - Metal Finisher SNC INFORMATION: • SIU was in SNC for the July-December period of 2023 for zinc violations (Daily max and Monthly avg.). Permit in process of renewal with a proposed modification to zinc limit and name change. ORDERS AND SCHEDULE INFORMATION: None A TO C AND CONSTRUCTION INFORMATION: None MISCELLANEOUS INFORMATION: • TTO Certificates statements were received for both halves of 2023. • Austin Co. IUP has been submitted for renewal review 1/24/2024. The Austin Co. had the following violation(s) for the January to June reporting period: • December 2022- NOV issued for 1 Zinc (monthly average) and 1 Zinc (daily max) a $400.00 fine issued. * Due to this NOV being issued in 2023, this NOV and fine was included in the 2023 PAR. • April 2023- NOV issued for late reporting. The Austin Co. had the following violation(s) for the July to December reporting period: • October 2023- NOV issued for 1 Zinc (monthly average) TRC and 1 Zinc(daily) and 2 daily Zinc TRC violations. $600.00 fine issued. • November 2023- NOV issued for 1 Zinc (monthly average) TRC and 1 Zinc (daily max) TRC. 500.00 fine issued. * Due to this NOV being issued in 2024, the NOV and fine will be included in the 2024 PAR. NOVs were sent for all violations. The Austin Co. was inspected by the Town's Pretreatment Department on 4/27/23 as part of a Pretreatment Audit. Flow meter calibrations were performed as required by permit for 2023. B&G Foods, Snacks IUP # 0004 — Bakery SNC INFORMATION: • SIU was not in SNC this PAR year (2023). ORDERS AND SCHEDULE INFORMATION: None. A TO C AND CONSTRUCTION INFORMATION: None. MISCELLANEOUS INFORMATION: Permit expires 5/31/25. B&G staff have been advised of upcoming renewal. B&G Foods, Snacks had the following violation(s) for the January to June reporting period: • February 2023- NOV issued for 1 flow violation. • March 2023- NOV issued for 1 flow violation. • April 2023--NOV issued for 1 flow violation and 2 copper violations. B&G Foods, Snacks had the following violations for the July to December reporting period: • July 2023- NOV issued for 6 pH violations. Violations were due to pH probe failure. • August 2023- NOV issued for 1 flow violation. 1 Oil and Grease violation (TRC). $ 100.00 fine issued. • September 2023- NOV issued for 1 flow violation and 1 TSS violation. $0.00 fine issued. • October 2023- NOV issued for 2 flow violations. No fine issued. B&G Foods has issues with flow mainly because of pieces of dough collect over time in their flow measurement equipment and cause the flows to read higher than they are. B&G Food staff have implemented more cleaning efforts into effect to improve the flow issues. B&G Foods, Snacks was inspected by the Town's Pretreatment Department on 9/28/2023. Flow meter calibrations were performed as required by permit for 2023. UNIFI, INC. UP # 0002 — Textile SNC INFORMATION : • SIU was not in SNC for this PAR year (2023). ORDERS AND SCHEDULE INFORMATION: None. A TO C AND CONSTRUCTION INFORMATION: None. MISCELLANEOUS INFORMATION: Permit approved for renewal 4/1/2024. Unifi, Inc had the following violations for the January to June reporting period: • February 2023 - NOV issued for 5 pH violations, violations occurred due to meter failure. These pH violations would continue into March. • March 2023 - NOV issued for 17 pH violations, violations occurred due to pH sensor failure. Unifi, Inc. had the following violations for the July to December reporting period: • NONE NOVs were sent for all violations. Unifi, Inc. was inspected by the Town's Pretreatment Department on 11/13/23. Flow meter calibrations were performed as required by permit. UNIFI, INC.
